Valganciclovir effective induction, maintenance therapy for CMV retinitis
Oral valganciclovir, the authors conclude, appears to be a convenient and effective treatment for induction and long-term maintenance of AIDS patients with CMV retinitis.
The proportion of patients in each arm with a "satisfactory" response to induction treatment was also similar--47 of 61 ganciclovir-treated subjects (77%) and 46 of 64 valganciclovir-treated subjects (71.9%).

In a 12 to 1 vote, the Antiviral Drugs Advisory Committee agreed that valganciclovir is safe and effective as induction therapy for CMV.
The therapy has triggered a "real paradigm change" in the way drugs for AIDS-related conditions are studied, noted panel chair Dr. Roger Pomerantz, professor at Philadelphia-based Thomas Jefferson University.

New Treatment Guidelines for Opportunistic Infections Recommend Valganciclovir for AIDS-related CMV Retinitis
Among patients with peripheral lesions that are not immediately sight-threatening, oral valganciclovir is preferable to the ganciclovir intraocular implant, intravenous ganciclovir, or intravenous foscarnet because of its greater ease of administration and lack of surgical or catheter-associated complications.
However, any of the treatment regimens can be chosen because epidemiologic studies and clinical trials have not demonstrated substantially reduced rates of loss of visual acuity among patients treated with the ganciclovir implant compared with those treated with systemic therapies.
The choice of initial therapy for CMV retinitis should be individualized based on the location and severity of the lesion(s), the level of underlying immune suppression, and other factors such as concomitant medications and ability to adhere to treatment.

Cytomegalovirus -- Facts, Info, and Encyclopedia article
Patients without CMV infection who are given organ transplants from CMV-infected donors should be given prophlyactic treatment with valganciclovir (ideally) or ganciclovir and require regular serological monitoring to detect a rising CMV titre, which should be treated early to prevent a potentially life-threatening infection becoming established.
Most infections with CMV are not diagnosed because the virus usually produces few, if any, symptoms and tends to reactivate intermittently without symptoms.
Whenever possible, patients without CMV infection should be given organs and/or blood products that are free of the virus.
