Topic: VDRL

Cecil Textbook of Medicine : />
Chronic, low-titer VDRL reactivity after therapy is much more common in cases of late syphilis and should not be viewed with alarm.
Patients with late latent syphilis should be examined also at 24 months after therapy; if CSF was not examined before therapy, a lumbar puncture should be done before discharge to rule out inadequately treated asymptomatic neurosyphilis.
In about 85% of patients with early (i.e., primary, secondary, or early latent) syphilis, quantitative VDRL titers become nonreactive in 12 to 24 months after therapy.

Its specificity is no better than that of the VDRL in healthy (99 percent) and sick (85 percent) patients, contrary to widespread belief.
If both tests are negative and the patient has a highly suspicious lesion (such as a painless genital ulcer), each can be repeated.
The specificity is about 99 percent and 84 percent in healthy and sick persons, respectively.

Comparative Study of VDRL and TPHA and their Complementary Roles in Serodiagnosis of Syphilis
Moreover TPHA should be done in blood transfusion products as VDRL may be weak positive or may be prozone phenomenon and be falsely interpreted as negative.
Three patients who were HIV +ve, out of the three one was found to have TPHA +ve (may be false positive) and two (2) were VDRL positive in lower titre (1:4).
Again the TPHA may be useful in concluding whether the infant should be treated or not.

Evidence of seropersistent VDRL test reactivity in an elderly Treponema pallidum-infected population
Nevertheless, it would be more correct to assess the old population of the infected with regard to previous treatment, or no treatment, by means of the Bayes' theorem-based procedure which we used in the No. 3 (1997) issue of this review, presenting the results of the first part of the same study (5).
Nevertheless, a growing trend in the number of VDRL nonreactors among the infected observed over 22 years points to the fact that in the past few years the treatment of syphilis has become increasingly effective.
Inserting our data on VDRL positive tests amounting to 49% (VDRL sensitivity) of all the infected (TPHA positives) into the Bayes theorem-based formula (5), together with VDRL sensitivity data for treated (1%) and untreated (70%) yielded a proportion of 0.7, an indication that 70% of our infected subjects had probably not been treated.

This usually includes a thorough physical examination for evidence of congenital syphilis and a quantitative nontreponemal serological test (VDRL or RPR) performed on infant serum (NOT cord blood).
On this basis, one could argue for treatment of all infants whose mothers at delivery had a positive VDRL, confirmed with a positive FTA-ABS.
The Laboratory Medicine resident will explain the procedure for sending the sample under these circumstances.

 2.1.4.2 Serological Testing
RPR titers are generally higher than VDRL titers, and are not directly comparable to VDRL titers to monitor response to therapy.
Nontreponemal tests (RPR, VDRL) are used to screen for syphilis and assess response to treatment.
Whereas serologically-defined failure may occur in up to 20% of HIV+ patients, clinically-defined failure has been reported but is thought to be much less common.
