We reserve fetoscopic cord coagulation in Twin-Twin Transfusion Syndrome for instances in which co-twin demise is imminent and fetoscopic laser surgery might adversely affect the available placental mass in the donor fetus predisposing to acute placental insufficiency and risking demise of the donor as well.
Until we have an effective medical therapy for Twin-Twin Transfusion Syndrome, a judicious application of invasive procedures should be employed to optimize risk:benefit ratios for the mother and fetuses.

Twin-to-twin transfusion syndrome (TTTS, also known as Feto-Fetal Transfusion Syndrome (FFTS) and Twin Oligohydramnios-Polyhydramnios Sequence (TOPS)) is a disorder that can affect identical twin or higher multiple pregnancies where two or more fetuses share a common (monochorionic) placenta.
The transfusion causes the donor twin to have decreased blood volume, retarding the donor's development and growth, and also decreased urinary output, leading to a lower than normal level of amniotic fluid (becoming oligohydramnios).
The oldest, most traditional treatment is through serial amniocentesis, which involves periodically draining fluid from the recipient twin in an effort to reduce the pressure of the amniotic fluid.
