Topic: Tuberculin

 TB Policies: Skin testing
The interpretation of the tuberculin skin test and all clinical decisions based thereon in an individual previously vaccinated with BCG should follow New York City Department of Health Chest Clinic criteria for reading a tuberculin skin test and indications for preventive treatment.
When a pregnant woman with a positive tuberculin skin test is referred from an outside clinic or agency for a chest x-ray, the chest x-ray should be performed with an appropriate lead shield in the NYCDOH Chest Clinic after the first trimester of pregnancy.
A negative tuberculin skin test result in a healthy individual who has no medical problems or is not a known contact of a case of active TB, is probably truly negative, and there is, therefore, no need to proceed with anergy testing.

 Tuberculin Testing and Preventative Therapy
Reinforce the fact that treatment decisions are based on risk benefit analysis: the risks of INH preventative therapy given the patient’s age versus the benefit of treatment given the length of time that the patient has likely been infected.
Discuss the fact that this patient would not be a candidate for INH preventative therapy because he does not fall into a high risk group, and the tuberculin test would not be considered positive.
Discuss the fact that despite the "positive" test, INH preventive therapy is not indicated due to the patient’s age and the absence of a documented negative tuberculin within the past two years.

The point of the needle is inserted into the most superficial layers of the skin with the needle bevel pointing upward.
The needle is then inserted gently through the cap and 0.1 mL of Tuberculin PPD is drawn into the syringe.
If the second result is negative, the person should be considered uninfected, and any positive reaction to subsequent skin test should be considered a true tuberculin skin test conversion.

AEGiS-MMWR: Targeted Tuberculin Testing and Treatment of Latent Tuberculosis Infection
Likewise, persons whose tuberculin skin tests convert from negative to positive within a period of 2 yr are presumed to have been infected recently.
Because of the risk for progressive and/or congenital TB, pregnant women who have a positive tuberculin skin test or who have negative skin-test results but who are recent contacts of persons with infectious TB disease should have chest radiographs (with appropriate shielding) as soon as feasible, even during the first trimester of pregnancy.
A flexible approach to identifying high-risk groups is recommended, and state and local public health agencies are encouraged to analyze their TB case reports and data obtained from tuberculin skin testing to identify high-risk groups based on local trends in the epidemiology of TB.

Because of the expectation that second strength (250 TU), PPD tuberculin, would be less sensitive than the other antigens in eliciting a DTH response, second strength PPD is not recommended for anergy evaluation; (nor is it recommended for evalua- tion for tuberculous infection).
