 Troponin: The Test
Troponin tests are primarily ordered for people who have chest pain to see if they have had a heart attack or other damage to their heart.
Troponin tests are usually ordered, often along with other heart tests such as CK, CK—MB, or myoglobin, when a patient has prolonged chest pain or other symptoms that could be related to heart injury.
Troponin is not generally affected by damage to other muscles so that muscle injections, accidents, strenuous exercise, and drugs that can damage muscle do not affect troponin levels.

Certain subtypes of troponin (Troponin I and T) are quite specific to the heart muscle (myocardium) and are measured in blood to differentiate between unstable angina pectoris and myocardial infarction in patients with chest pain.
In the absence of calcium, tropomyosin interferes with this action of myosin, and therefore muscles remain relaxed.

Troponins are components of muscle contractile structure along with actin and tropomyosin.
Troponin results must be taken in context with the patient's clinical circumstances.
It should be noted that troponin I and T are not "early markers" of cardiac events and a negative result shortly after the onset of chest pain is of no more value than a negative CK-MB.

The pathway cannot be described in detail here, but involved electrocardiograms, clinical examination and judgement, and the use of troponin I, CK-MB and myoglobin tests done using a point of care testing machine at presentation and 30, 60 and 90 minutes later.
One of the problems with new diagnostic tests is getting one's brain around what they mean and how and when to use them.
