The aim of this study was to determine the etiology and to evaluate the clinical consequences of an extreme thrombocytosis.
ETiology and Incidence of Thrombotic and Haemorrhagic Disorders in THAI PATIENTS with Extreme thrombocytosis
Subtypes of our MPD cases were chronic myeloid leukemia (30/56,53.6%), essential thrombocytosis (18/56,32.1%), polycythemia vera (4/56,7.1%), agnogenic myeloid metaplasia (3/56,5.4%) and unclassified MPD (1/56,1.8%).

Reactive thrombocytosis has been reported to occur as a response to various conditions and surgical procedures.
In cardiac surgery performed with extracorporeal circulation reactive thrombocytosis is generally considered to be a benign transient condition, although cases with postoperative complications connected with reactive thrombocytosis have been described.
This study has again established that reactive thrombocytosis frequently occurs after coronary artery bypass surgery and that it does not routinely require any specific therapy.

For this reason, in our study, peripheral and bone marrow SCF levels were investigated in patients with thrombocytosis and thrombocytopenia.
Regarding to the bone marrow SCF measurements, mean SCF concentration of the thrombocytosis group (2694 ± 267 pg/ml) was again greater than the mean SCF concentration of the thrombocytopenic group (1700 ± 185 pg/ml) (p < 0.05).
Our results indicate that SCF is operative in the pathological megakaryopoiesis of clonal and reactive thrombocytosis both in the local bone marrow microenvironment and peripheral circulating blood.

"Now that we know that surgery is less likely to remove all the visible cancer in patients with thrombocytosis and that these patients' cancers are more likely to recur, we can individualize their treatments or use experimental therapies, which may improve their long-term survival," said Dr. Karlan.
"Our findings show that thrombocytosis correlates with a more aggressive tumor biology in ovarian cancer and a poorer response to treatment," said Dr. Beth Karlan, Medical Director of the Women's Cancer Research Institute at Cedars-Sinai Medical Center and senior author of the study.
"These findings indicate that thrombocytosis is somehow linked to more aggressive ovarian cancer," said Dr. Karlan.

Thrombocytosis is a condition in which the body produces excess number of platelets.
(2) Secondary thrombocytosis can be caused by other health problems, such as: chronic inflammation, severe bleeding, iron deficiency, stress, surgery, medication, osteoporosis, splenectomy, anemia, polycythemia vera (splenomegalic polycythemia, Vaquez-Osler syndrome, erythremia), and cancer.
Some patients may die as a result of blood clots or uncontrolled bleeding but many patients with thrombocytosis remain symptom free for long periods of time.

Thrombocytosis is also found in the other myeloproliferative disorders i.e.
Thrombocytosis (an increased platelet count) can either be reactive (secondary to an infectious or inflammatory process, iron deficiency, trauma, bleeding, surgical or functional asplenism or malignancy) or primary due to overproduction of platelets due to a myeloproliferative disorder.
In primary thrombocytosis there can also be a modest increase in eosinophil or basophil numbers which is not a common feature in reactive thrombocytosis.
