Topic: TSH

 Medical Conditions - Thyroid
TSH assays have been classified by "generation" based on the functional sensitivity of the assay, i.e., the lowest concentration at which the assay is able to maintain an interassay precision, expressed in terms of percent coefficient of variation (%CV), of 20% or less.
Patients with these conditions appear to have physiologically inactive TSH in their circulation that is recognized by TSH assays to a degree such that they may yield misleading, "euthyroid" TSH results.
Normally, TSH would rise into the concentration range measurable with less sensitive TSH assays that could provide useful information from the profile of increase even if they were not sensitive enough to measure baseline values.

Physiological Control of TSH in Man
The diurnal rythmicity of serum TSH concentration is maintained in mild hyper- and hypothyroidism, but is abolished in severe short-term primary hypothyroidism, suggesting that the complete lack of negative feedback to the hypothalamus or pituitary or both may override the central influences on TSH secretion.
The circadian variation of TSH secretion is probably the consequence of a varying dopaminergic tone modulating the pulsatile TSH stimulation by TRH
For example, starvation is a severe stress and markedly reduces TSH secretion in rats, but only marginally in humans.

 Levoxyl® - TSH Test Information
The adequacy of therapy is determined by periodic assessment of appropriate laboratory tests and clinical evaluation.
In patients with congenital hypothyroidism, the adequacy of replacement therapy should be assessed by measuring both serum TSH (using a sensitive assay) and total- or free-T
therapy and/or of the serum TSH to decrease below 20 mU/L within 4 weeks should alert the physician to the possibility that the child is not receiving adequate therapy.

Control of TSH by Drugs and Disease
In contrast to findings in rats, there is scant evidence of an adrenergic control of TSH secretion in humans.
Fasting-induced changes in dopaminergic tone do not seem to be sufficient to explain the TSH changes
This is one of the few instances where there is not a close correlation between basal TSH levels and the response to TRH (see below).

However, indications of the unreliability of the TSH test keep coming to light in medical studies.
The references in this section are limited to those about problems with and within the typical TSH reference range of around 0.3 to 5.5 mU/L. The fact that a TSH level above the reference range correlates with hypothyroidism is recognized in medical circles and doesn't need further documentation.

With the sensitive TSH test, however, physicians are able to diagnose thyroid disorders at an earlier stage -- in many cases, even before patients begin to experience symptoms.
The task force also suggested that, in patients on thyroxine replacement therapy, "The target TSH level should be between 0.3 and 3.0 uIU/mL." By inference, any patient with TSH outside this range has an "abnormal" level which should be carefully followed but not necessarily treated.

tsh is secreted by the pituitary gland and is not only affected by the synthroid you are taking but is also affected by another organ called the hypothalamus that secretes another substance that controls the release of tsh.
By the way it takes about 6 weeks for the effect of the synthroid dose change to appear on the tsh level in your blood, so if you had the test before that, it might have not reflected the true effect.

The b subunit of TSH and the b subunits of the related hormones are structurally hormone-specific, and confer upon them their unique biological activities.
