 Evaluation of New Oral Hypoglycemic Agents
Of the 2,287 patients in the 6-year cohort, 676 were allocated to conventional therapy with 1,611 to intensive therapy (922 allocated to sulfonylurea and 689 to insulin therapy).
Additionally, 685 obese subjects were allocated to intensive therapy (consisting of 393 allocated to sulfonylurea and 302 allocated to insulin therapy).
Long-term, well-designed trials are necessary to determine if metformin should replace sulfonylureas as first-line therapy in nonobese patients, especially in the absence of hyperlipidemia.

There are various pharmacological approaches to improving glucose homeostasis, but those currently used in clinical practice, either do not succeed in restoring normoglycemia in most patients, fail after a variable period of time or have side effects that ultimately preclude their use in many patients.
(Surprisingly, the dose-response relationships of the sulfonylureas have been poorly investigated.) By complementing and efficiently optimizing the therapeutic action of sulfonylurea, the formulations of this invention permit the use of minimal doses of sulfonylureas, thereby lowering the risks of sulfonylurea therapy, including hypoglycemia.
The formulations of this invention provide appropriate therapeutic levels of a sulfonylurea and will enhance and/or extend the beneficial effect of the sulfonylureas upon plasma lipids, coagulopathy and microvascular permeability.
