Reducing the Odds: Preventing Perinatal Transmission of HIV in the United States
Disease screening is one of the most basic tools of modern public health and preventive medicine.
To carry out this report, the Institute of Medicine established a committee of 13 individuals, with expertise in pediatrics, obstetrics and gynecology, preventive medicine, women's health, and other relevant medical specialties; social and behavioral sciences; public health practice; epidemiology; program evaluation; health services research; bioethics; and public health law.
Thus access to care, the potential for ready implementation of screening along with other prenatal testing, and the availability of therapy to improve the outcomes of both mothers and infants in the face of HIV infection, all have led the committee to focus on this episode of care.

 Harvard Medicine Research: Cancer
Widespread use of a similar pap smear screening technique would allow detectionóand early removalóof pre-cancerous anal lesions among high-risk HIV negative men.
Screening for colorectal cancer is as cost effective as other forms of cancer screening, and deaths from the disease can be significantly reduced with even a single colonoscopy at age 55, according to a study.
Study suggests that laparoscopyóexamination of the abdomen with an endoscope-like instrument through a small incisionócan be useful in classifying good candidates for pancreatic cancer surgery and guiding their treatment.

Thus, current screening practices miss iron deficiency in non-anemic infants in whom adverse consequences may be developing.
"Our findings are important because, while iron deficiency can be readily treated, practitioners haven't had a simple, reliable and practical screening test to detect it early enough; now they might," said Henry Bernstein, DO, Associate Chief, General Pediatrics, Children's Hospital Boston and the principal investigator of the study.
Other tests can diagnose iron deficiency in the absence of anemia but are considered impractical for routine clinical screening.

 All About Blood
During the period from 1986 to 1988, the percentage of reported cases related to blood transfusion declined to 1.0 percent, possibly as a consequence of the donor screening questions that were instituted to identify persons at increased risk for HIV infection.
Transfusion medicine specialists are continually researching new technologies to further reduce the transmission of HIV.
This procedure is widely used for surgical procedures, such as cardiac, vascular, orthopedic, urologic, trauma, gynecologic, and transplant surgery, in which the anticipated blood loss is 20 percent or more of the patient's estimated blood volume and there is no contamination of the area by bacteria or cancer cells.

News and Information - Study Finds Colonoscopy Best Choice for Screening
"While fecal occult blood testing and flexible sigmoidoscopy are less expensive, faster, and require no sedation, 65 percent of women with advanced precancerous polyps in our study would have lesions missed if these were the only screening tests performed because precancerous polyps are found deeper in the colon in women," Dr. Schoenfeld reports.
The polyp can then be sent to a lab to be checked under a microscope to see if it has any areas that have changed into cancer.
In response to the new research, the ACS program director of the prostate and colorectal cancer control department, Dr. Durado D. Brooks, says, "This is an important and somewhat concerning study.

 Postgraduate Medicine: Men's health
This symposium focuses on three men's health issues commonly encountered in the primary care office: erectile dysfunction, prostate cancer screening, and testosterone deficiency.

 GenomicsTrainingConference2002 Family History
Evaluation of family history: Before family history can be embraced as a public health screening strategy, the accuracy and reliability of the family history tools used to stratify disease risk should be assessed, and the effectiveness of the risk stratification on early detection and prevention efforts should be determined.
Although family history has been shown to be a risk factor for many chronic diseases, including coronary heart disease, diabetes, cancer, osteoporosis, and asthma, the collection and interpretation of family history has rarely been applied in the preventive medicine setting to assess disease risk or to influence early detection and prevention strategies.
Family history of specific diseases reflects the consequences of genetic susceptibilities, shared environment, and common behaviors.

Health-IT World: Is Mass Spec Testing on Newborns Bad Medicine?
Clayton argues that many states are being forced to adopt mass spec from the fear of losing their newborn screening businesses to the private sector.
"The reason people are moving to tandem mass spec is they want to detect MCAD - medium chain acyl-CoA dehydrogenase deficiency [inability to breakdown fat] - and it turns out to be ideal to do that," Clayton says.

Chromosome 'caps' predict bone marrow disease
Reporting in Nature Genetics today (18 April 2004), the Hammersmith team, collaborating with scientists at the Washington University School of Medicine in the USA, is hoping that the findings could lead to better screening for this and other diseases, as well as suggest targets for new therapies.
"The findings of this study provides a novel mechanism for disease anticipation in humans," explains Professor Dokal.
For the first time, Imperial College London researchers at the Hammersmith Hospital studying a rare bone marrow disease have found an association between 'telomere shortening' - changes in the lengths of DNA repeats at the end of chromosomes - and the time of development and severity of disease symptoms in patients.

A transfusion medicine scientist in the USA who is very knowledgeable about blood donor screening technologies reports that in her opinion, there is no current "resolution" to the issue raised by the Denmark colleague.
A third transfusion medicine scientist in the USA who is also very knowledgeable about blood donor screening technologies reports that in his opinion the serological failure to detect HBsAg mutants is an unresolved problem, especially for blood banks that do not use anti-HBc testing in routine screening of donated blood.
A second transfusion medicine scientist in the USA who is also very knowledgeable about blood donor screening technologies reports that in his opinion, it may be unrealistic to expect that there will be tests that detect ALL mutants of HBsAg.
