Topic: Resection

 Short Bowel Syndrome
Intestinal resection also leads to a series of morphologic and functional changes of the bowel, which aid in the recovery of bowel function, termed intestinal adaptation.
This section will discuss the features of normal digestion and absorption and the physiologic consequences of intestinal resection to demonstrate how best to care for these patients.
This technique is especially useful in patients who have previously has small bowel resection and are at risk for the SBS.

eMedicine - Transurethral Resection of the Prostate : Article by Stephen W Leslie, MD, FACS
If the transurethral resection will take longer than 90 minutes of operating time to complete, then an open prostatectomy procedure or some alternative therapy is recommended.
Resectionists throughout the country discovered that the operation could not be performed with ease; that its technique was exceedingly difficult to acquire as well as to execute; that the incidence of morbidity and mortality could be alarmingly high following transurethral resection; and that unexpectedly poor end results were observed in a disconcerting number of patients.
The precise distance between the verumontanum and the external sphincter demonstrates some slight individual variation and should be verified visually before starting the resection and periodically during the surgery.

 Resection Margins
Resection margin is a surgical factor that has been evaluated for its influence on the long-term outcome after resection of HCC, but its significance remains controversial.
Adjuvant therapy has so far proved disappointing in preventing recurrence after resection of HCC, and aggressive management of recurrence appears to be the best way of improving the long-term outcome.
Based on a prospectively collected database, we conducted a detailed analysis of the effects of the width and histologic involvement of the resection margin on the incidence and pattern of recurrence after resection of HCC.

 Pancreatic Cancer
For the surgical treatment of pancreatic head cancer, the classical Whipple operation is still the standard procedure but during the last two decades, pylorus-preserving duodenopancreatectomy has been evolved as a more conservative procedure in order to omit the consequences of partial gastrectomy.
However, due to the resistance of pancreatic cancers against chemoradiation, curative resection represents the only therapy with a potential for cure.
Finally, only surgical exploration can offer full opportunity for resection for cure.

 Acoustic Neuroma: Resection or Radiosurgery
Surprisingly little information has been published on long-term imaging-based outcomes after resection.
Post et al found that 4 of 56 patients (7%) had an incomplete resection in their attempted hearing preservation series and that three developed regrowth within three years ( 24).
Younger patients with good hearing should have attempted hearing preservation, either with radiosurgery or resection ( 28).

Other patients present with a past medical history of intestinal ischemia from malrotation and volvulus that required intestinal resection.
Intestinal adaptation may take weeks to months to be achieved, and, in the interim, children who have had intestinal resection need nutritional support through a variety of therapeutic measures, including parenteral nutrition.
As a consequence, the infant and the young child have a favorable long-term prognosis compared to an adult in regards to potential intestinal growth after intestinal resection.

Resection of Sympomatic, Complex Aspergilloma - Kucharczuk
These results are probably due to a combination of better patient selection, modified operative techniques, an improved postoperative management.
Surgical resection remains the definitive treatment of symptomatic complex aspergilloma.  Improvement in patient selection, operative techniques, and postoperative care has markedly improved the outcome in these very difficult patients.
Our recent institutional experience with surgical resection for complex pulmonary aspergilloma includes 22 consecutive patients over a thirteen year period.  Preoperative FEV1 ranged from 20% to 80% of predicted.
