eMedicine - Nephrolithiasis: Acute Renal Colic : Article by Stephen W Leslie, MD, FACS
The classic presentation for a patient with acute renal colic is the sudden onset of severe pain originating in the flank and radiating inferiorly and anteriorly.
Colic is a misnomer because renal colic pain tends to remain constant, whereas intestinal or biliary colic is usually somewhat intermittent and often comes in waves.
Nausea is caused by the common innervation pathway of the renal pelvis, stomach, and intestines through the celiac axis and vagal nerve afferents.

This calcium channel interaction is of particular interest when considering its potential role in renal colic, since calcium channel antagonists such as nifedipine have been shown to relieve the pain and promote passage of renal calculi through a vasodilatory effect on the ureter smooth muscle.
All that is required to practice the method of this invention is to administer a GABA analog in an amount that is effective to treat pain associated with renal colic.
Thus, gabapentin could have a dual mechanism in the relief of acute renal colic through its interference with central and peripheral pain pathways in addition to its potential to provide ureter smooth muscle relaxation.

Physical: The classic patient with renal colic is writhing in pain, pacing about, and unable to lie still, in contrast to a patient with peritoneal irritation, who remains motionless to minimize discomfort.
Ureteral obstruction stimulates synthesis of prostaglandin E2 in the renal medulla, which increases ureteral contractility and renal blood flow, leading to increased ureteral pressures and painful renal colic.
Potential adverse effects on renal function, GI mucosa, and platelet aggregation do not appear clinically important when used for short-term pain relief.

The name 'renal' colic can be misleading because the pain does not emanate from the kidney itself.
Renal colic is the name given to an intermittent, but often very severe, abdominal pain which is most commonly caused by a tiny stone passing down the ureter, which is the tube which connects the kidney to the bladder.
The usual course is for the pain to begin in the side or upper abdomen, and travel down to the lower abdomen.
