Topic: Remifentanil

 Remifentanil
Among the opioids, remifentanil behaves like it has an "on and off switch" and the practitioner needs to be aware of five ways in which the patient can manifest undesirable effects and for which anesthesiologists and anesthetists must be prepared.
It is most important that the anesthesiologist using remifentanil plan in advance for the management of postoperative pain for patients who are expected to experience it.
(2) The bradycardia and hypotension produced by remifentanil are muted or prevented by pretreatment with an anticholinergic such as 0.4 mg glycopyrolate intravenously, and both can be treated by a sympathomimetic drug such as ephedrine.

REMIFENTANIL: Clinical use of an evanescent opioid.
Remifentanil is in the same structural family as fentanyl and the other phenylpiperidines.
Remifentanil is 15 times more potent than alfentanil, based on the ED50 to achieve loss of response to a verbal command and 20 times more potent than alfentanil based on the EC50.
In another open, prospective trial, 28 patients were randomly allocated to receive continuous infusions of remifentanil (6 mg/kg/min) or propofol (180 mg/kg/min) for sedation during spinal or axillary regional anesthesia.

A New Approach For Induction Anaesthesia With Propofol Target Controlled And Remifentanil
Remifentanil and alfentanil are the faster onset of the opioids in clinical practice, but the incidence of bradicardia, hypotension and rigid chest are the main problems with them.
Educational considerations for the clinical introduction and use of remifentanil.
Bolus dosis of remifentanil can cause hypotension, rigid chest and bradicardia, because is difficult do it slow(90 or 120 seconds) without pump infusion.

Gallileus - Biomedcentral Open Access Articles
The proportions of patients experiencing pain during the maintenance phase were similar in the two groups, with 51% of patients in the remifentanil group having at least moderate pain as compared with 49% of patients in the fentanyl group.
Rapid offset of analgesia with remifentanil resulted in a greater incidence of pain, highlighting the need for proactive pain management when transitioning to longer acting analgesics, which is difficult within a double-blind study but would be quite possible under normal circumstances.
A further 26 patients (who all received open-label remifentanil; referred to hereafter as 'practice patients') were treated at the remaining sites (up to 2 patients per site) to allow familiarization with the protocol procedures.
