NIH Guide: PROTEASE INHIBITOR RELATED ATHEROSCLEROSIS IN HIV INFECTION
Combinations of various protease inhibitors with nucleoside and non-nucleoside reverse transcriptase inhibitors usually have synergistic effects and are used effectively in clinical practice.
Proposed Research The known information on the dyslipidemias and lipodystrophy associated with protease inhibitor therapy for HIV-positive patients is derived from clinical observations.
Four protease inhibitors are currently in use: saquinavir mesylate, ritonavir, indinavir sulfate and nelfinavir.

SF AIDS Fdn: BETA 9/97 -- Protease Inhibitor Drug Interactions
Stopping a protease inhibitor may result in the development of drug resistance and may preclude later use of that drug.
Much is already known about protease inhibitors and the ways they affect and are affected by other drugs.
Ketoconazole (Nizoral) and related drugs block the breakdown of saquinavir, indinavir and perhaps other protease inhibitors, with a resulting increase in protease inhibitor levels.

SF AIDS Fdn: BETA Live! 8/97 -- Protease Inhibitor Drug Interactions
But it’s very important that people taking protease inhibitor drugs be aware that any other drug they can take could affect the activity of their protease inhibitor.
We’ll be discussing with our guests how protease inhibitors interact with each other, with other classes of anti-HIV drugs and with other types of drugs that are commonly used by people living with HIV and AIDS, such as antihistamines, antinausea drugs, antidepressants and many more.
Included in those drug interactions are the protease inhibitors, because protease inhibitors can inhibit the breakdown of the antihistamines, leading to high serum levels.

USC HIV/AIDS researcher announces new protease inhibitor
Lipodystrophy is a common problem linked to protease inhibitors in which a patient’s belly swells or a pad of fat may grow behind the neck, while the arms, legs and face become gaunt.
"Our study showed that BMS-232632 is a potent protease inhibitor that is well tolerated and can be used safely in combination with other anti-HIV therapies," said Kathleen Squires, M.D., associate professor of medicine at the Keck School of Medicine of USC.
About 17 percent of patients taking BMS-232632 in the study experienced diarrhea, for example, far fewer than the 55 percent of patients taking the other protease inhibitor.

HIV & AIDS - JAMA study shows increased mortality from protease inhibitor use
These two studies did not use a true placebo, but instead placed one group on two drugs plus a protease inhibitor and the other group just on the two other drugs.
For example, the first study to be reviewed found that 40% of people on any drug regimen had to change drugs, mostly due to toxic side effects.
The study opens with the usual claims that "Triple drug combination therapy has been shown to dramatically decrease morbidity and mortality" (page 2568), and cites the two randomized studies mentioned at the opening of this message as evidence.

Eight Years of HIV Protease Inhibitor Therapy:
The assumptions underlying this change are worth re-examining in light of improvements in protease inhibitors that have made them both simpler and more efficacious.
For example, data from ACTG 5095 as well as the recently discontinued ESS30009 study indicate that triple nucleoside therapy may not be sufficiently potent to be recommended as stand-alone treatment.  Moreover, the resistance consequences of failing some triple nucleosides regimens can be particularly problematic.
Thus, there has been a resurgence of interest in the proper role of PIs in HIV treatment, and the remainder of this essay will focus on how the more recently developed drugs appear to fit into current HIV treatment approaches.

Long-term antiretroviral drug treatment including protease inhibitors increases HIV-positive men's risk for heart attack, according to a study published in the Nov. 21 issue of the journal
In an accompanying editorial, Dr. Peter Reiss of the University of Amsterdam, writes, "It seems prudent to at least consider individually managing patients with respect to their risk of developing coronary artery disease," adding that doctors should encourage HIV-positive patients to stop smoking and appropriately modify diet and exercise (
Although the life expectancy gained by taking antiretroviral treatments that include protease inhibitors "far outweighs" the increased heart attack risk, physicians should consider patients' cardiovascular disease risk factors when deciding whether to prescribe protease inhibitors, the researchers concluded.

CROI: HIV in Patients with Multiple Protease Inhibitor Mutations Remain Susceptible to Tipranavir
"Baseline phenotypic susceptibility to tipranavir was maintained in 69.4% of HIV-1 isolates that were already highly resistant to available protease inhibitors," Dr. Cooper reported.
Conference on Retroviruses and Opportunistic Infections, researchers said that 93% of the patients recruited into their study had 10 or more mutations that compromised treatment with available antiretroviral drugs, and 41 of the isolates from the cohort of 216 patients had genotypes that were indicative of resistance to all currently marketed protease inhibitors.
Cooper's study examined the extent of susceptibility that the treated patients exhibited despite the multidrug-resistant profile of the virus.
