Topic: Polysomnography

 Perineuro: Polisomnografia
Polysomnography is indicated when evaluating patients with sleep behaviors suggestive of parasomnias that are unusual or atypical because of the patient's age at onset; the time, duration, or frequency of occurrence of the behavior; or the specifics of the particular motor patterns in question (e.g.
Polysomnography may be indicated when the presumed parasomnia or sleep-related epilepsy does not respond to conventional therapy.
Parasomnias are undesirable physiologic phenomena that occur predominantly during sleep.

 NGC - NGC Summary
Polysomnography is indicated when initial diagnosis is uncertain, treatment fails (behavioral or pharmacologic), or precipitous arousals occur with violent or injurious behavior.
For this reason, polysomnography is a component of the standard procedure used to verify insomnia and assess treatment efficacy for research purposes.
Exclusion criteria were: 1) reports of treatment outcome studies, 2) reports of ambulatory polysomnography (PSG) and specialized waveform analysis, 3) reports that did not present original data (editorials, letters, and reviews), and 4) single case studies.

The majority of the comments in favor of using multi-channel home sleep study testing as an alternative to a polysomnography in a facility-based sleep study laboratory came from medical institutions/centers, physicians, respiratory therapist, laboratories, industry representatives and associations.
Results were reported for 20 subjects who underwent simultaneous portable studies and laboratory based polysomnography and 5 subjects who underwent separate unattended portable studies and laboratory based polysomnography.
Polysomnography monitors sleep stages, respiratory effort, oxygen saturation, heart rate, body position, and limb movements.

Can Home Sleep Studies Replace Polysomnography?
Because polysomnography is closely monitored, technical problems can be corrected while the study is performed, thereby preventing the need to repeat it.
However, Nancy A. Collop, MD, believes that polysomnography is still superior, a position she defended in a pro–con debate at the 67th Annual International Scientific Assembly of the American College of Chest Physicians in Philadelphia.[1]
Furthermore, data have suggested that a large proportion of patients prefer this device to polysomnography performed in a sleep laboratory.

 AARC Addresses Polysomnography Issues
AARC holds the position that polysomnography is a subspecialty of the respiratory care scope of practice.
Ensuring appropriate accreditation for polysomnography educational programs took center stage in late July when the AARC took part in a meeting held by the Committee on the Accreditation of Allied Health Education Programs (CAAHEP) to develop an accreditation system for the polysomnographic field.
The Association will be working closely with these other groups over the coming year to work out an approach to polysomnography program accreditation that will be acceptable to all concerned.
