Topic: Pituitary adenoma

 Pituitary Tumors
Pituitary adenomas are benign tumors (meaning that they are non-cancerous and do not travel to other parts of the body) that arise exclusively within the anterior pituitary.
Large adenomas may also present with headaches due to: 1) the adenoma pressing against nearby brain structures, 2) an increase in intracranial pressure (the pressure within the skull, which can compress the brain if the pressure increases), or 3) a blockage of the ventricles within the brain causing hydrocephalus.
Pituitary tissue is acquired through biopsy at the time of surgical removal.

Pituitary Tumor, Pituitary Adenoma, Dr. Jho's Endoscopic Pituitary Surgery
The conventional surgical methods for removal of pituitary adenomas involve incisions under the upper lip or in the nostril.
endoscopic pituitary surgery is performed through a natural nasal air pathway without any incisions (unlike the conventional microscopic surgery performed with an incision made under the upper lip or inner aspect of a nostril).
Although this patient was referred for an invasive craniotomy surgery due to the hard fibrotic nature and large suprasellar protrusion of the tumor, endoscopic transspehnoidal surgery via a nostril was performed instead.

The symptoms of a pituitary adenoma depends on what hormone it secrets.
Pituitary adenomas used to be classified by the color of their cytoplasm, acidophilic, chromophobe and basophilic, but these colors give no clue as to which hormone is being secreted.
•Cytoplasm may be acidophilic, pale pink or basophilic but usually is of only one color in a single adenoma.

ituitary adenomas typically exert pressure on the chiasm in the midline from below.
The exact sites of nerve fiber compression may show some variation, depending on whether the chiasm is prefixed (has short optic nerves), in the middle position (by far the commonest case), or postfixed (with long optic nerves).
Ventral crossed fibers, which originate from the inferior nasal retinas, are thus the ones that show the earliest signs of involvement, accounting for bilateral superior temporal defects in the visual fields (superior bitemporal quandrantanopia).
