Persistent vegetative state - Wikipedia, the free encyclopedia
Patients in a persistent vegetative state are usually considered to be unconscious and unaware.
There is controversy in both the medical and legal fields as to whether this condition is irreversible.
This typology distinguishes various stages of the condition rather than using one term for them all.

 Persistent Vegetative State
This state can last for more than fifteen years, however the longer a patient is in PVS the less likely they are of recovering from it.
PVS patients are treated to prevent infections, bed sores, and with physical therapy to prevent contractures and orthopedic disformities.
This can be observed because persons in a vegetative state are unable to track moving objects or fixate their vision on an object, and as a patient recovers they regain this ability.

Coma and Persistent Vegetative State Information Page: National Institute of Neurological Disorders and Stroke (NINDS)
coma is a profound or deep state of unconsciousness.
The NINDS supports and conducts research on neurological conditions such as coma and persistent vegetative state.
Physical therapy may also be used to prevent contractures (permanent muscular contractions) and deformities of the bones, joints, and muscles that would limit recovery for those who emerge from coma.

Catholic Culture : Document Library : Caring for Persons in the Persistent Vegetative State
The "persistent vegetative state" is a form of deep unconsciousness.
However, their condition has stabilized and they are not in any immediate danger of death so long as they are given appropriate "food," which can be provided to them "artificially" by means of tubes.
First of all, such persons, even including those in the "persistent vegetative state," can be fed orally provided that the swallowing reflex is sufficiently unimpaired.

 Coma - vegetative state
Coma is a state of unconsciousness that is similar to deep sleep, except no amount of external stimuli (such as sounds or sensations) can prompt the brain to become awake and alert.
A person in a coma can't even respond to pain.
This includes the regulation of many functions essential to life such as heart rate, respiration and blood pressure.

THE PERSISTENT VEGETATIVE STATE AND THE WITHDRAWAL OF NUTRITION AND HYDRATION
As stated above, the PVS individual is not dead in the holistic physiological sense.
Because the neocortical concept of death involves a major redefinition of death—something that the whole brain death concept does not do since it simply recognizes what always occurs at death—the scientific community has held back from accepting neocortical destruction as a sufficient definition of death.
Finally I will ask whether neocortical destruction may be equated with the death of the person and what response or responses may be morally acceptable in light of the findings obtained.

The researchers were unable to identify "predictors of recovery from the "vegetative state" -- that is, there is no established test by which we can tell in advance which PVS patients will ultimately wake up.
Locked-in State - An article in Psychology Today, Sept/Oct 2002 by Patton Howell and James Hall tells the story of a patient suffering from a "locked-in" state.
As the health care profession moves rapidly towards a quality of life ethic, many coma patients may be written off as hopeless too soon.

Responding to Patients in the Persistent Vegetative State - an ethical and legal dilemma.
A central problem in the MSTF argument is that it equates the lack of behavioral evidence for consciousness with the absence of consciousness.
The MSTF reiterated this position: "Patients in a coma are unconscious because they lack both wakefulness and awareness.
Patients in a vegetative state are unconscious because, although they are wakeful, they lack awareness."

It also has been suggested that these criteria be reviewed through neurologic examinations repeatedly.

Additionally, the condition of being in a vegetative state is similar to that of other conditions that involve the absence or partial absence of awareness of self and the environment, such as coma, locked-in syndrome and akinetic mutism.

 WMA - Policy
Persons with overwhelming damage to the cerebral hemispheres commonly pass into a chronic state of unconsciousness called the vegetative state in which the body cyclically awakens and sleeps but expresses no behavioral or cerebral metabolic evidence of possessing cognitive function or of being able to respond in a learned manner to external events or stimuli.
Abrupt loss of consciousness usually consists of an acute sleep-like state of unarousability called coma that may be followed either by varying degrees of recovery or severe, chronic neurologic impairment.
This condition of total cognitive loss can follow acute injuries causing coma or can develop more slowly as an end result of progressive structural disorders, such as Alzheimer's disease, that in their end stages also can destroy the phychological function of the cerebrum.

Because their brain stem is still intact, people in a vegetative state can follow things with their eyes, but only slightly to the left or to the right, Bernat said.
To make a diagnosis of persistent vegetative state, Bernat said, "there really has to be zero evidence of any responsiveness that suggests awareness."

It can appear as if you are aware, but it's a state of unconsciousness." How can a patient be awake but unaware?
Accepting that a loved one is brain-damaged beyond recovery, that he or she is in a "persistent vegetative state," incapable of thought or emotion, is all but impossible for many people, for reasons that go deeper than the fear of loss.
A lifetime of intimate familiarity with a person, unresolved feelings of guilt, psychological projection of personal needs - all of these conspire to reject a hopeless diagnosis, no matter how medically certain it may be, psychologists say.

In 1995 the AAN published a practice parameter about the assessment and management of patients in the persistent vegetative state.
The AAN Ethics, Law, and Humanities Committee (formerly known as Ethics and Humanities Subcommittee) published an approved position paper in 1989 about certain aspects of the care and management of patients in the persistent vegetative state.
The American Academy of Neurology (AAN) has received many inquiries from the press regarding the definition of persistent vegetative state in light of the Terri Schiavo case.

Meanwhile, Dr. William Cheshire of the Mayo Clinic in Jacksonville, also believed Terri is minimally conscious rather than a PVS patient.
Fins indicated Terri's condition amy have been misdiagnosed and that she was more likely in a state of minimal consciousness rather than a PVS patient as courts and many media outlets have alleged.

The vegetative state needs considerable skill to diagnose, requiring assessment over a period of time; diagnosis cannot be made, even by the most experienced clinician, from a bedside assessment.
