Topic: Pegylated

Postgraduate Medicine: Current therapies for chronic hepatitis C
In the interim, pegylated interferon and ribavirin remain the cornerstone of therapy.
Most patients who receive combination therapy with pegylated interferon and ribavirin have an early reduction in HCV RNA levels.
This approach may be appropriate for nonresponding patients with advanced fibrosis or cirrhosis and is being evaluated in three large, multicenter trials.

PharmaCare - HCV Medication pegylated interferon (PEG-Intron)
Overall, clinical studies have found that pegylated interferon used in combination with ribavirin (Rebetol®, Copegus®) is more effective than standard interferon in combination with ribavirin or pegylated interferon used alone.
This means that the pegylated interferon stays in the body longer and is able to fight off the HCV virus more effectively.
Therefore, standard interferon plus ribavirin may be an effective therapeutic option for individuals with genotypes 2 or 3.

News - Pegylated Interferon Induces Sustained Responses in Asymptomatic Patients With Hepatitis C Infection: Presented ...
"Pegylated interferon leads to higher sustained viral response rates in symptomatic patients with acute HCV infections, but the treatment of asymptomatic HCV is not yet established," said Sanaa Kamal, MD, professor of medicine, Harvard University, Boston, Massachusetts.
Continuation of therapy for another 12 weeks raised the early virologic response rate in the interferon-ribavirin group to 74.2% and in the pegylated interferon group to 96.7%.
Patients in each of these groups were randomly assigned to 12 weeks of therapy with interferon alfa-2b plus ribavirin or once-weekly pegylated interferon alfa monotherapy.

 Combo to Pegalayted
First of all, it is not clear that pegylated interferon alone will be significantly better than combination therapy, which is the combination of standard interferon (3X a week) and ribavirin.
I do advise some of my patients who have early levels of fibrosis on biopsy that there is little risk to waiting six to 12 months to see how these drugs continue to do in clinical trials and to see when they are likely to be released.
A second point I would make is that pegylated interferon is still a form of interferon and there are still considerable side effects.

Clinical Care Options for Hepatitis - Pegylated Interferon and Ribavirin: Also the Standard of Care for HIV-infected ...
While future studies are ongoing to clarify these issues, it is now clear that pegylated interferon and ribavirin therapy is the treatment of choice for HIV/HCV-coinfected patients.
However, of the 5 instances occurring in persons taking pegylated interferon and ribavirin, 4 were in patients who should not have been enrolled in the study, because they had baseline Child-Pugh scores of more than 5.
Each study contributed one other finding that I believe is important with regard to the effectiveness of treatment.

 News Review
Combination therapy with pegylated interferon plus ribavirin for 24 to 48 weeks can result in eradication of the virus in about 50 percent of patients, he adds.
Most genotype 1 patients fail initial therapy of pegylated interferon and ribavirin and become non-responders.
Non-responders to previous hepatitis C therapy are the subjects of SciClone's two U.S. phase 3 clinical trials for ZADAXIN in combination therapy with pegylated interferon.

Aidsmap Treatment for HIV/HCV coinfection: three major studies report at CROI
Overall, 40% of coinfected patients treated with pegylated interferon/ribavirin achieved SVR, compared with 20% of those receiving pegylated interferon monotherapy and 12% of those receiving standard interferon/ribavirin (corresponding end-of-treatment response rates were 49%, 33%, and 14%).
Within the pegylated interferon arm, 80% with genotypes other than 1 showed an end-of-treatment response, compared with 29% of genotype 1 patients.
Eligible participants either had a CD4 count of at least 100 cells/mL, an HIV viral load less than 10,000 copies/mL, and stable antiretroviral therapy for at least 12 weeks, or else were antiretroviral naive and had a CD4 count of at least 300.

 Conference Reports
Cost of treating genotype 1 patients with interferon (pegylated and non) an ribavirin was analyzed.
Some patients with a poor response to IFN achieve on-treatment virological responses to pegylated IFN/ribavirin, although relapse is not uncommon after the end of treatment.
Eradicating HCV in the majority of infected patients remains an elusive goal, particularly in previously treated patients who failed or relapsed following prior therapy.

This study focused on those with paired liver biopsies (the first biopsy was done within 12 months of starting therapy and the second biopsy was done after treatment) which were reviewed by a central liver pathologist.

This indicates the need for more research side-effect management related to both interferon and ribavirin.

HepNet: Results of the Pegylated Interferon Trial
Comparison of these preliminary data with data on interferon/ribavirin suggest that monotherapy with pegylated interferon will not be as effective as the current standard of care, combination therapy using interferon a-2b/ribavirin.
These trials demonstrate that pegylated interferon is more effective than interferon monotherapy.
Compared with standard interferon, pegylated interferon appears to be better tolerated, more convenient and, therefore may well have a role as therapy for those who cannot tolerate interferon a-2b/ribavirin or maintenance therapy.

CHEST: Pegylated interferon and ribavirin-induced interstitial pneumonitis with ARDS - selected reports
Interstitial pneumonitis has been previously described as a complication of interferon-[alpha] therapy (1-6) but, to our knowledge, never has been reported with pegylated interferon-[[alpha].sub.2]b therapy following its approval by the US Food and Drug Administration in January 2001.
This ease suggests that interstitial pulmonary disease can occur with pegylated interferon-[[alpha].sub.2]b therapy.
A possible pulmonary allergic reaction to pegylated interferon-[[alpha].sub.2]b was suspected as the cause of his symptoms, and the patient therefore was started on therapy with methylprednisolone, 60 mg IV q8h.

 Pegylated Interferon
Combination therapy with peginterferon and ribavirin is the most effective treatment for patients with chronic hepatitis C. Long-term response rates appear to be similar for the two combinations of pegylated interferon and ribavirin.
Approximately 24 percent of treatment-naive people who take either form of pegylated interferon for a period of 48 weeks demonstrate sustained virological response- that is, their HCVRNA is nondetectable (less than 50 IU/mL) six months after completion of therapy.
Response rates, dosage of ribavirin and length of therapy vary with HCV genotype as follows:
