Determination of the amylase isoenzyme level is useful in distinguishing effusions caused by pancreatic disease (pancreatic isoamylase) from effusions caused by esophageal rupture or nonpancreatic carcinoma (salivary isoamylase).
Pleural effusions associated with breast carcinoma typically are ipsilateral to the primary breast cancer (58-70%); however, 20-26% develop effusions contralateral to the primary lesion, and 10-16% develop bilateral effusion.
Isolated left-sided effusions occur with esophageal rupture, pancreatic disease, subphrenic or splenic abscess, splenic infarction, diaphragmatic hernia, pericardial disease, or coronary artery bypass surgery.

The distinction of choriocarcinoma from undifferentiated pancreatic carcinoma may be impossible by cytomorphology alone.
However, the differential diagnosis of choriocarcinoma from a poorly differentiated pancreatic carcinoma, particularly undifferentiated (anaplastic) carcinoma may be very challenging, even more so if the typical biphasic pattern of syncytiotrophoblast and cytotrophoblast cells of choriocarcinoma is absent or unappreciated, as was the case in this patient.
One pure choriocarcinoma of the bladder has been reported to have the i(12p) by FISH analysis, but no details of the probes used for such analysis were provided by the authors.

 Pancreatic Cancer
Despite the low overall incidence of resectability and postoperative cure rate for pancreatic carcinoma, exploration for potential curative resection should not necessarily be withheld for healthy, selected patients who are older than 70 years.
For the surgical treatment of pancreatic head cancer, the classical Whipple operation is still the standard procedure but during the last two decades, pylorus-preserving duodenopancreatectomy has been evolved as a more conservative procedure in order to omit the consequences of partial gastrectomy.
However, due to the resistance of pancreatic cancers against chemoradiation, curative resection represents the only therapy with a potential for cure.

Another group presented two patients with symptomatic duodenal obstruction from carcinoma of the pancreas who failed endoscopic therapy, but were palliated subsequently by duodenal Wallstents inserted perorally under fluoroscopic guidance.
Duodenal obstruction develops in approximately 5-15% of pancreatic cancer patients treated by endoscopic biliary decompression.

Both AFP and HCG are not tumour specific as AFP are raised in NSGCT and hepatocellular carcinoma, but also in gastric, colon, biliary, pancreatic and lung cancers while HCG levels may be increased in pancreatic adenocarcinoma and islet cell tumours, tumours of the small and large bowel, hepatoma, stomach, ovarian, breast and renal cancer.
The presence of yolk sac tumour elements is associated with raised serum alpha-foetoprotein (29), while embryonal carcinoma is regarded as more aggressive (30).
There is also the reported negative correlation between the histological finding of yolk sac tumour elements and occult metastases in clinical stage I patients (31).
