Clinical and Molecular Allergy Full text Low-dose, long-term macrolide therapy in asthma: An overview
Low-dose, long-term macrolide antibiotics therapy may have effects beyond their corticosteroid-sparing action in asthma.
Spector and his colleagues [14] conducted a double-blind crossover trial comparing troleandomycin to placebo in 74 corticosteroid-dependent patients with severe asthma and chronic bronchitis.

This paradox appears to have a rational explanation based upon the mechanism of low level macrolide resistance (concentration-dependent efflux) and the ability of the macrolides to be concentrated upon epithelial surfaces in the respiratory tract at levels exceeding the maximal MIC of the efflux strains of pneumococci.
There is emerging evidence that the macrolides do not perform equally in achieving pulmonary concentrations adequate to treat efflux-mediated resistance–a topic which will be explored in upcoming features as the data are published.
Thus the clinical paradox seems to have a coherent explanation emerging from an understanding of microbiologic resistance mechanisms and the pharmacologic distribution of macrolides.

Macrolide resistance in Streptococcus pneumoniae: Fallacy or fact?
Questions arise as to whether the in vitro susceptibility results of macrolides against S pneumoniae are relevant with respect to clinical efficacy and outcomes, and whether macrolides should continue to be the initial treatment of choice for CAP.
There are several arguments in favour of the clinical relevance of in vitro macrolide resistance that come from both laboratory and clinical data.
The debate will continue with respect to the clinical relevance of macrolide resistance in pneumococci until carefully conducted prospective studies determine the level at which in vitro resistance to macrolides translates into clinical failure.

It is our hope that the map we have presented will stimulate awareness of the situation, prompt further studies for both a wider and deeper appreciation of this new problem in the epidemiology of resistance and promote continued international collaboration to address it.
What emerges from this survey is the scarcity of available information about the resistance phenotype (even though some data about clindamycin resistance seem to show that the M phenotype prevails in most countries), and the extreme scarcity of molecular studies performed to date.
Moreover, erythromycin resistance is not always tested in routine laboratory practice because penicillin has remained the drug of choice for group A streptococci, and the consumption of new macrolides is almost nil.

However, increased macrolide use has been associated with a global increase in pneumococcal resistance, which is leading to concern over the continued clinical efficacy of the macrolides to treat community-acquired RTIs.
Data reported after completion of the first year of the PROTEKT study (1999–2000 respiratory season) confirmed this high global incidence (31.0%) of pneumococcal macrolide resistance (7), with similar overall levels of resistance among isolates collected as part of the PROTEKT US study (31.0% in 2000–2001 and 27.9% in 2001–2002) (16,17).

Adequate actions for control are strongly needed in both veterinary and human medicine.
Multidrug resistance to macrolides and fluoroquinolones must be considered highly undesirable in Campylobacter as these two classes are generally advocated as first- and second-line drugs for antimicrobial treatment of Campylobacter enteritis.
As Campylobacter infections can be serious in immunocompromised patients, the identified treatment failure raises the concern that fluoroquinolone-resistant strains may increase Campylobacter-associated deaths in this group of patients.
