Intravascular ultrasound - Wikipedia, the free encyclopedia
Arguably the most valuable use of IVUS has been in research to better understand the behavior of the atherosclerosis process in living people.
Additionally, IVUS examinations, as they were done more frequently, especially at research centers, served to reveal and confirm the autopsy research findings published by Steven Glasgov, M.D. in 1987, that most of the atherosclerotic process, contrary to many popular illustrations, exists primarily outside the lumen and is not recognized on angiography examinations.
Additionally, IVUS also facilites much more accurate measurement of the degree of stenosis of the opening of the coronary arteries than possible with angiography.

IVUS therefore, is of considerable importance in assessing the mass of myocardium at risk, prognosis, and in the selection of intracoronary therapeutic intervention.
Linear regression analysis was used to estimate the accuracy of both techniques (IVUS and QCA) in quantification of atheromatous lesions.
For quantitative coronary cineangiographic analysis, the cardiovascular measurement system: medical imaging system, Nueren, Netherlands was used.

 Cardialysis - Fostering Discoveries
The analysis of baseline and follow-up IVUS studies in the individual patient can provide information regarding not only lesion volume but also changes in plaque composition.
Recently, the Cardialysis IVUS Core Laboratory has acquired some novel IVUS derived imaging techniques such as Echogenicity, Palpography and Virtual Histology.
The underlying principle is that soft material will deform more readily than hard material when force is applied to the tissue.

Intravascular Ultrasound (IVUS) Imaging of Coronary Arteries
The use of IVUS to guide stent placement has now obviated the need for thrombolytic therapy.
Therefore, increasing the acute procedural success, as measured by the minimal luminal diameter or the lumen cross-sectional area, has been investigated as a risk factor for future restenosis and as a tool to guide further patient management.

Carrier IVUS policies are likely to change, and services such as peripheral studies are likely to be covered by more payers in the future.
The situation is much the same for IVUS on peripheral vessels: Most carriers consider the service “investigational” and do not cover it.
A review of Medicare carriers’ local medical review policies (LMRPs) indicates that many carriers continue to cover IVUS when performed with therapeutic coronary interventions only.

"VH(TM) IVUS is the first IVUS system capable of providing information in the cardiac catheterization laboratory about the plaque composition," said Martin B. Leon, MD Chairman of the Cardiovascular Research Foundation(R) and the Associate Director of the Center for Interventional Vascular Therapy (CIVT) at Columbia University Medical Center, New York City.
Volcano's new VH IVUS technology uses advanced spectral analysis techniques to allow simplified interpretation of ultrasound images and provide detailed information on the composition of each patient's atherosclerotic plaques.
By simplifying IVUS use and by providing important new information to the interventional cardiologist, we are confident that VH IVUS will improve the treatment of patients and help to further expand IVUS use in percutaneous coronary interventions."

Coronary heart disease - Wikipedia, the free encyclopedia
High grade stenoses exceeding 75% blockage, such as detected by stress testing, were found to be responsible for only 14% of acute heart attacks.
The typical narrowing of the lumen of the heart artery before sudden closure is typically 20%, according to clinical research completed in the late 1990s and using IVUS examinations within 6 months prior to a heart attack.
Myocardial infarction usually results from the sudden occlusion of a coronary artery when a plaque ruptures, activating the clotting system and atheroma-clot interaction fills the lumen of the artery to the point of sudden closure.

In some cases, cardiologists are also using a new catheter-based procedure called intravascular coronary ultrasound (IVUS) to examine the extent of the blockage, and help them choose the right size balloon and stent to be used during angioplasty.
These include lasers, a rotating, diamond-encrusted burr that pulverizes plaque, and another device that shaves and deposits plaque into a capsule.
In approximately one-third of all cases, the blockages return and the artery becomes narrow again.
