 pg33
Boyle and associates used the hypoglycemic glucose clamp in combination with measurements of cerebral blood flow and brain arteriovenous differences to examine these issues in human subjects.
One of the major obstacles in studying counterregulatory hormone responses to hypoglycemia was the lack of methods that would allow investigators to produce a controlled and reproducible hypoglycemic stimulus in vivo.
As expected, glucose levels triggering hormonal responses and perception of hypoglycemic symptoms were significantly lower in intensively-treated patients as compared to their poorly-controlled counterparts, and hormonal responses were suppressed as compared to healthy controls.

Diabetes: Phlorizin Significantly Increases Glucosuria in Normoglycemic Rats but not in Zucker Diabetic Fatty or Streptozotocin-Induced Diabetic Rats - ZDF - STZ
The purpose of the current study was to further characterize the hypoglycemic and glucosuric effects in diabetic and nondiabetic rats, with the hypothesis that the glucosuria may not be the causal factor for hypoglycemia after administration of PZ.
Previous studies have suggested that hypoglycemic effects of phlorizin (PZ) can be attributed to glucosuria.
In PZ treated fa/fa rats, glucosuria occurred within 1h, reached a plateau at 4h, and was maintained for at least 12h, but was not accompanied by a hypoglycemic response as in the ZDF or STZ diabetic rat models.

BioMed Central Full text Identifying older diabetic patients at risk of poor glycemic control
Indeed, physicians are expected to encounter greater difficulty in tailoring the hypoglycemic therapy to the needs of patients in more unstable conditions and to compensate them more cautiously.
These variables assess health status, type of hypoglycemic therapy prior to admission, comorbidity and severity of the metabolic derangement, as expressed by selected laboratory and clinical parameters.
The allocation to a ward of Geriatrics or General Medicine was unrelated to the outcome glycemic control, whereas the use of insulin alone or with oral hypoglycemic agents prior to admission, diastolic hypotension on admission and a Charlson's index greater than 2 were more common among patients having fasting glycemia ≥ 140 mg/dl at discharge.

UCSF News Office - Experimental drugs prevent neuron death in rats following insulin shock
This is the stage of hypoglycemia known as hypoglycemic shock or coma, or insulin shock, and when it occurs, a biochemical chain reaction is initiated that ends with the destruction of neurons, particularly in the hippocampus, a region of the brain instrumental in processing memories.
To test whether they can also protect brain cells from insulin shock, he and his team examined the effects of the drugs in brain cell cultures derived from mice, as well as on rats subjected to induced hypoglycemic coma.
The destructive biochemical pathway triggered by insulin shock shares some similarities to pathways that damage heart and brain cells as a result of heart attack and stroke.

 Mayo pediatric study/tight control
For each follow-up, insulin requirements, levels of glycosylated hemoglobin, and frequency of hypoglycemic reactions were analyzed for three age-groups--0 to 2 years, 2 to 5 years, and 5 to 9 years old.
OBJECTIVE: To assess the level of glycemic control and to determine whether more normal glycemic control, as measured by glycosylated hemoglobin, leads to frequent hypoglycemic episodes in young patients with type 1 diabetes mellitus.
RESULTS: The mean glycosylated hemoglobin for the first 2 years after diagnosis of type 1 diabetes was higher in children 0 to 2 years old in comparison with the other age-groups.

Improved blood glucose-lowering effect of NovoLog Mix 70/30 compared to Lantus
Major hypoglycemic events were rare, with no major events reported in the NovoLog® Mix 70/30 group and one observed in the insulin glargine group.
In the study, hypoglycemic events were the most common side effect reported in both groups.
Average A1C value (the measurement of a person's blood glucose level over the preceding two to three months) in the NovoLog® Mix 70/30 group was significantly lower than in the insulin glargine group after 28 weeks for the 233 patients enrolled in the study (6.9 percent vs. 7.4 percent, p=0.001).

The parties should be free to argue evidence to allow the trier of fact to determine whether defendant was suffering from a hypoglycemic condition, whether that condition impaired his ability to conform his conduct, and whether the circumstances show that this condition was voluntarily induced.
Defendant endorsed a proposed expert, an endocrinologist, to testify to the effects of hypoglycemia, to defendant’s historic hypoglycemic condition, and to state his opinion that, in a state of hypoglycemia, defendant could not conform his conduct to the law.
A defense based upon mental defect or insanity must be affirmatively pleaded at arraignment, in part because it sets in motion a specific process by which the defendant is professionally evaluated.

Many of the people who have corresponded with the organizer of this Web site have reported the problem of reactive hypoglycemia - that is, the tendancy of their body to generate its own hypoglycemic symptoms without the use of exogenous insulin or oral agents.
It must be repeated that this item is an anecdotal report of the experiences of two individuals, and is not necessarily indicative of a valid approach to the problem of reactive hypoglycemia.

Experimental Drugs Reduce Neuron Death In Rats Following Insulin Shock
Six weeks after these tests, rats that had been immediately treated with PARP inhibitors were able to complete memory and learning tests just as well as rats that had not been subjected to hypoglycemia, while rats in the control group (those that had not received PARP inhibitors following hypoglycemic coma) fared poorly in the tests.
For the rat tests, Swanson and his colleagues devised several scenarios that simulate the kinds of circumstances that might occur when a person goes into hypoglycemic shock.
Rats that received the drug one hour after the end of glucose deprivation experienced about 70 percent reduction in cell damage, and rats that received the drug two hours afterwards experienced about 50 percent reduction in cell damage.

Long-term outcomes (survival and occurrence of microvascular complications) of treatment of DM are related to the degree of glycemic control achieved, but not to the means used to achieve control (diet/exercise vs. oral hypoglycemic agent vs. insulin, or any known combination therapy).
