Topic: Hypocalcemia

eMedicine - Hypocalcemia : Article by Alfredo A Pegoraro, MD
Hypocalcemia within the first 3 weeks of life has been observed in infants of mothers with hyperparathyroidism.
Hypocalcemia also may occur in infants born to mothers with hyperparathyroidism.
Patients with postparathyroidectomy hungry bone disease, especially those with osteitis fibrosa cystica, can present with a dramatic picture of hypocalcemia.

So… Despite the importance these authors seem to place upon hypocalcemia as a potential diagnosis for late-gestation does that are down, none of them give us any clinical signs with which to identify it, nor any information on how to treat it, nor (equally as important) any suggestions on how to prevent it!
No calcium, no muscle tone, no heart pumping, dead goat.
(Just as others have, they referred to hypocalcemia as eclampsia.) While they don't spell out why it happens, they do observe that a calcium deficiency (hypocalcemia) sometimes exists in the pregnant/lactating goat, and that the owner should be prepared to treat it when it shows up.

This situation occurs particularly after surgical correction of hyperparathyroidism in patients with severe osteitis fibrosa cystica and has been termed the hungry bone syndrome.
Gallium nitrate infusion is indicated after failure of saline and loop diuretics to control malignancy-associated hypercalcemia, although its effectiveness with repeated use is not established and long-term therapy requires further evaluation.
Muscle weakness and pain and typical bone deformities can occur.

These changes lead to hypocalcemia, hypokalemia, hypomagnesemia, hypophosphatemia, and alkalemia, which mimic intestinal obstruction except for the diarrhea.
Hypophosphatemia is a condition in which there is too little phosphorus in the blood.
Treatment involves feeding roughage and no grain, electrolytes and fluid therapy, anti-inflammatories, and nursing care.

Be aware that severe hypocalcemia often is associated with other life-threatening conditions.
The patient's past medical history should be explored for pancreatitis, anxiety disorders, renal or liver failure, gastrointestinal disorders, and hyperthyroidism or hyperparathyroidism.
The incidence of ionized hypocalcemia is difficult to quantify, but it has been reported to be 15-50% for ICU patients.

 Hypocalcemia
The cause of this hypocalcemia (calcium deficiency) problem that can show up in a doe anytime in the last 6-8 weeks of pregnancy is very basic.
Hypocalcemia is a correctable metabolic disorder (condition), and not a disease.
Hypocalcemia (calcium deficiency) is a serious condition in which the calcium that a doe needs to support herself and provide for the needs of her developing fetuses (or to produce milk if she is lactating) is unavailable to her because of incorrect feeding.

This condition might be encountered in association with a number of medical problems but is most commonly diagnosed in the newborn.
Very low calcium levels can cause marked jitteryness or even convulsive seizures; this most commonly occurs with infants of diabetic mothers.
