The editorial in the Annals of Internal Medicine advocates glycosylated hemoglobin, in part because it is "inexpensive".
Every one percent increase in glycosylated hemoglobin above 5 percent increased risk of heart attack or stroke by 20 percent.
Those men and women who had glycosylated hemoglobin levels between 5.1 percent and 6.9 percent had a consistent and progressive increase in the rate of heart attack, stroke, and death as the glycosylated hemoglobin level rose from 5 percent to 5.5 percent to 6 percent to 6.5 percent and to 7 percent.

The hemoglobin A1c values were used in assessing glycemic control and modifying therapy in one of the two groups.
The proportion of patients with poor control, defined as those having a hemoglobin A1c value above 10.0 percent, decreased from 46 to 30 percent (P less than 0.01) in the group whose hemoglobin A1c level was monitored but did not change significantly (45 to 50 percent) in the control group.
The value of routine measurements of glycosylated hemoglobin (hemoglobin A1c) in the care of patients with diabetes mellitus is uncertain.

In the absence of well-controlled studies that suggest a definite testing protocol, expert opinion recommends glycosylated hemoglobin be obtained at least twice a year in patients who are meeting treatment goals and who have stable glycemic control and more frequently (quarterly assessment) in patients whose therapy was changed or who are not meeting glycemic goals.

 Hemoglobin
While your daily blood testing tells you how your blood sugar is doing right then, allowing you to make necessary changes in medicine, food and exercise, it doesn’t give you a picture of your long-term diabetes management success.
Sugar that is not used for energy is left in the blood, where it attaches itself to the hemoglobin, which is the part of the red blood cell that carries oxygen, in a process called glycosylation.
The glycosylated hemoglobin test, or Hemoglobin A1c (HbA1c), is a test used to give you and your doctor the most accurate picture of your overall diabetes control.

Currently many patients and physicians attempt to do better than that.
Accepted "target levels" of glucose and glycosylated hemoglobin that are considered good control have been lowered over the last 25 years, because of improvements in the tools of diabetes care, because of increasing evidence of the value of glycemic control in avoiding complications, and by the expectations of both patients and physicians.
In nondiabetic persons with normal glucose metabolism the glycosylated hemoglobin is usually 4-6% by the most common methods (normal ranges may vary by method).
