As an embryo doesn't express the blood group until about the same stage as the heart beat is seen on ultrasound scan, then pregnancies which don't reach this point don't (theoretically) warrant anti-D. As a blighted ovum doesn't even reflect an embryo(= anembryonic gestation), anti-D shouldn't be needed.

Evidence supporting the diagnosis of anembryonic gestation would be a clinical history of a more advanced gestational age than what is seen, the presence of bleeding and cramping, or abnormalities of the gestational sac itself.
Anembryonic gestation is suspected if no yolk sac is seen when the gestational sac has reached a mean diameter of > 10 mm, or when no fetal pole is seen when the gestational sac is greater than 18 mm in mean diameter.

The study involved 652 women with early pregnancy failure (due to anembryonic gestation, embryonic or foetal death, or incomplete or inevitable spontaneous abortion) who were randomised 3:1 to receive treatment with intravaginal misoprostol or vacuum aspiration (as the current standard of care).
If the first misoprostol dose was not completely effective, the dose was repeated on the third day; if this was ineffective, vacuum aspiration was used.
A randomised trial has shown that intravaginal misoprostol has a high success rate in early pregnancy failure and is well tolerated.

Nulliparous women who had singleton gestation in vertex presentation, spontaneous onset of labor at 37-42 weeks, who gave birth from October 1, 1992 to September 30, 1993, and from October 1, 1995 to September 30, 1996 were selected from a computerized obstetric data base.
The study population consists of 800 pregnant women who have an incomplete spontaneous abortion, anembryonic gestation, embryonic/fetal demise or inevitable abortion in the first trimester.
The subjects will be randomly assigned to receive either misoprostol 800ug per vagina or D&C. All subjects will complete daily diaries regarding bleeding, pain and other symptoms until 2 weeks after initial treatment.
