Topic: Gallbladder cancer

 Gallbladder Cancer
Chemotherapy for cancer of the gallbladder is usually put into the body by a needle inserted into a vein.
The chance of recovery (prognosis) and choice of treatment depend on the stage of cancer (whether it is just in the gallbladder or has spread to other places) and on the patient's general health.
Radiation for gallbladder cancer usually comes from a machine outside the body (external-beam radiation therapy).

Gallbladder Diseases: Research Looks to More Effective Treatments
Researchers have developed new methods of treating gallbladder cancer during the last few years, but surgical removal of the gallbladder (a procedure known as cholecystectomy) remains the most popular therapy.
Radiation for gallbladder cancer usually comes from an external machine (external-beam radiation therapy).
Gallbladder cancer is the fifth most common cancer of the gastrointestinal tract; about 7,000 people are diagnosed with the disease each year, and about 3,500 people die of gallbladder cancer each year.

 Gallbladder cancer
Gallbladder cancer is often misdiagnosed because it mimics other more common conditions, such as gallstones, cholecystitis, and pancreatitis.
Larger gallstones are associated with a higher chance of developing gallbladder cancer.
Current chemotherapy or radiation therapy cannot cure gallbladder cancer, but they may offer some benefit in certain patients.

Gallbladder cancer, Ellis Fischel Cancer Center, University of Missouri Health Care
It is important that patients with gallbladder cancer be evaluated by a surgeon with experience treating this condition, since surgery is the only potentially curative therapy.
In cases where gallbladder cancer is found prior to surgery, radical cholecystectomy is recommended.
When unsuspected cancer is found in the gallbladder wall after simple cholecystectomy, reoperation to resect part of the adjacent liver and the nearby lymph nodes may improve survival.

 Gallbladder cancer
Although most people with gallbladder cancer also have gallstones, the vast majority of people with gallstones never develop gallbladder cancer.
Although the exact cause of gallbladder and bile duct cancers isn't clear, researchers believe that DNA in the cells of your biliary tract may be damaged by toxins that are routinely metabolized by your liver.
Diagnosing gallbladder cancer earlier than this is difficult because the gallbladder is hidden behind the liver and so is relatively inaccessible, because signs and symptoms don't develop until late in the disease, and because when symptoms do appear, they can easily be mistaken for those of many other conditions.

 Gallbladder Cancer
People don't think about their gallbladder unless it is diseased with gallstones, or it develops cancer.
There has been much research in the past decade on treating gallbladder and other biliary tract cancers.
In the past gallbladder cancer was almost always fatal, since it tended to be advanced when first be diagnosed and effective treatments were not available.

 Gallstones and Gallbladder Disease
The primary advantages of surgical removal of the gallbladder over nonsurgical treatment are both the elimination of gallstones and also the prevention of gallbladder cancer.
This procedure uses a needle to withdraw (aspirate) fluid from the gallbladder.

 Gallbladder removal
Gallbladder removal is surgery to remove the gallbladder if it is inflamed or obstructed, if gallstones are causing pancreatitis, or if cancer is suspected.
Gallbladder removal (cholecystectomy) is done while the patient is under general anesthesia, unconscious and pain-free.
Gallbladder removal is usually done to treat the following conditions:

 Pancreatic Cancer
Cancer of the pancreas is difficult to diagnose because the organ is surrounded and hidden by other organs, including the stomach, small intestine, bile duct, gallbladder, liver and spleen.
When a cancer spreads from its origination to other areas of the body it is still referred to by its point of origin.
Cancer can arise in many sites and behaves differently depending on its organ of origin.

National Cancer Institute - Pancreatic Cancer Treatment
The pancreas is hidden behind other organs such as the stomach, small intestine, liver, gallbladder, spleen, and bile ducts.
Pancreatic cancer can be controlled only if it is found before it has spread, when it can be removed by surgery.
Smoking and health history can affect the risk of developing pancreatic cancer.

 Liver cancer
Because the liver is close to a number of significant organs — including the pancreas, gallbladder, stomach, colon, breasts and lungs — and because the liver is richly supplied with blood, it's especially vulnerable to metastatic tumors.
This means the cancer has returned to your liver or to another part of your body after being treated.
Standard therapies for adult liver cancer primarily include surgery, radiation therapy and chemotherapy.

ACS :: What Is Gallbladder Cancer?
Gallbladder cancers are graded by how closely they look like normal (noncancerous) gallbladder tissue when viewed under a microscope.
Typically, low-grade cancers are less likely to spread outside the gallbladder than high-grade cancers, and have a more favorable outlook.
About 6% of all gallbladder cancers are papillary adenocarcinomas, and almost all the rest are mucinous adenocarcinomas, adenosquamous carcinomas, squamous cell carcinomas, or small cell carcinomas.

Immunohistochemical demonstration of cytokeratin is useful for detecting micrometastatic foci from gallbladder carcinoma in regional lymph nodes -- Tajima et al. 29 (9): 425 -- Japanese Journal of Clinical Oncology
Results: All 78 lymph nodes from 12 patients with early gallbladder carcinoma were negative for both HE- and cytokeratin-positive cancer cells.
Therefore, accurate staging of lymphatic spread in patients with gallbladder carcinoma is essential for predicting the surgical outcome.
Carcinoma of the gallbladder usually has a dismal outcome, even after surgery.

National Cancer Institute - Genetic Testing for BRCA1 and BRCA2
Alterations in the BRCA2 gene have also been associated with an increased risk of lymphoma, melanoma, and cancers of the pancreas, gallbladder, bile duct, and stomach in some men and women.
Women with an inherited alteration in one of these genes have an increased risk of developing these cancers at a young age (before menopause), and often have multiple close family members with the disease.
Inherited alterations in the genes called BRCA1 and BRCA2 (short for breast cancer 1 and breast cancer 2) are involved in many cases of hereditary breast and ovarian cancer.

Gene mutation boosts cancer risk in men
A previous study suggested that carriers of mutant BRCA2 genes are at increased risk of cancer of the prostate, pancreas, gallbladder, bile duct and stomach, as well as malignant melanoma, breast cancer and ovarian cancers (Journal of the National Cancer Institute, vol 91 p 1310).
Using information from known mutated-BRCA2 carriers in these families, the researchers studied the retrospective incidence of cancers among both male and female family members with a 50% chance of being a carrier – amounting to 1811 people.
Christi van Asperen and her colleagues at the Centre for Human and Clinical Genetics at Leiden University in The Netherlands speculated that estimates of cancer risk at other sites in the body may differ in mutated-BRCA2 carriers with less striking, though still present, family histories of cancer.

gms Integrated Positron-Emission Tomography and Computed Tomography (PET/CT) is Highly Sensitive for Gallbladder Cancer but not for Carcinomas of the Extrahepatic Bile Duct System
The study population was composed of 10 patients with Klatskin tumor, 7 with gallbladder cancer, 4 with intrahepatic cholangiocarcinoma and 4 with extrahepatic bile duct cancer.
All 7 patients with gallbladder cancer had a high specific FDG uptake into the tumor demonstrating a sensitivity of PET/CT of 100%.
High specific FDG uptake of the tumor was observed in 3/4 intrahepatic cholangiocarcinomas (75%), 1/4 extrahepatic bile duct cancers (25%), and 4/10 Klatskin tumors (40%).
