Eighteen patients with meningioma of the ventral foramen magnum were treated surgically in three neurosurgical departments at the University of Mississippi Medical Center (1990), Loyola University Medical Center (1991--1993), and the University of Arkansas for Medical Sciences (1993--1998).
Meningiomas of the ventral foramen magnum are one of the most challenging types of meningiomas to treat because a large number of vital neurovascular structures, which are sensitive to injury, are crowded in this deeply hidden central area.
The authors conducted a retrospective study of 18 patients who harbored lesions in the ventral foramen magnum (mean follow up 40 months) in whom surgery was performed via a transcondylar approach.

The absence of a csf signal dorsal to the ectopic cerebellar tonsils, or distortion of the brain stem, cervical spinal cord or cerebellar tonsils was taken as evidence of physical compression of the contents of the foramen magnum.
Radiographic evidence of tonsillar ectopia in the absence of a convincing clinical picture was not sufficient to prescribe surgical therapy.
Despite the minimal degree of tonsillar ectopia, our patients reported improvement in symptoms in response to surgical enlargement of the foramen magnum.

As a result, spinal CSF is driven into the spinal cord through the perivascular spaces to form a syrinx.

A sagittal MRI is a much easier method of determining if foramen magnum herniation is present as in this patient with a tentorial subdural and shift through the foramen magnum.
This is postulated to occur as the anterior spinal tracts within the ventral spinal cord, become compressed against the bone of the posterior vertebral bodies.
Patients with a Chiari malformation may have little or no clinical symptomatology or may demonstrate Lhermitte’s phenomenon.

In anatomy, the foramen magum is the large hole through the occipital bone in the base of the skull, through which the medulla oblongata (an extension of the spinal cord) exits the skull vault.
Comparisons of the position of the foramen magum in early hominid species are useful to determine how comfortable a particular species was when walking on two limbs (bipedality) rather than four.

AllRefer Health - Increased Intracranial Pressure (ICP, Intracranial Pressure - Increased)
The pressure itself can be responsible for further damage to the central nervous system by decreasing blood flow to the brain or by causing the brain to herniate (push through) the opening in the back of the skull where the spinal cord is attached.
Many conditions can cause increases in the intracranial pressure including metabolic, toxic, traumatic, infectious, neoplastic and idiopathic.
Sudden herniation through the foramen magnum (back of the skull) is fatal.
