Filariasis is characterized by fever, chills, headache, and skin lesions in the early stages and, if untreated, can progress to include gross enlargement of the limbs and genitalia in a condition called elephantiasis.
The best method of preventing filariasis is to prevent being repeatedly bitten by the mosquitoes that carry the disease.
In Bancroftian filariasis, the legs and genitals are most often involved, while the Malayan variety affects the legs below the knees.

Given the medical need for ivermectin to treat lymphatic filariasis in countries where there is concomitant onchocerciasis or loaisis, Merck decided to expand the Mectizan Donation Program beyond the treatment of onchocerciasis.
While diethylcarbamazine alone has been used for more than 50 years to treat lymphatic filariasis, recent studies indicate that a combination of albendazole and DEC or albendazole and ivermectin is more effective, reducing microfilarial level in the blood by 99 percent.
The decision to expand the Mectizan Donation Program comes after research conducted by Merck in collaboration with the World Health Organization that demonstrated that Mectizan is effective against lymphatic filariasis.

Further clinical trials are essential for strengthening the evidence base on the role of this drug in the morbidity management of lymphatic filariasis.
When affected by lymphoedema, they are considered undesirable and when their lower limbs and genital parts are enlarged they are severely stigmatized; marriage, in many situations an essential source of security, is often impossible.

Background: Mass treatment with albendazole, co-administered with another antifilarial drug, is being promoted as part of a global programme to eliminate lymphatic filariasis.
Authors' conclusions: There is insufficient reliable research to confirm or refute whether albendazole alone, or co-administered with diethylcarbamazine or ivermectin, has an effect on lymphatic filariasis.

Ottesen emphasizes that long-term clinical research projects such as Dr. Bockarie's are critical to measuring LF elimination efforts and making adjustments as the program moves into countries with new challenges.
Ongoing scientific research must accompany treatment, however, if the elimination program is to continue to succeed in additional countries and affected populations, according to an editorial in the Dec. 5 issue of the New England Journal of Medicine.
"Were it not for the earlier successes of research in lymphatic filariasis, the global elimination program could not have been launched," Dr. Ottesen says.
