PA-03-049: CHEMICAL SCREENS FOR NEW INDUCERS OF FETAL HEMOGLOBIN (SBIR/STTR)
The induction of fetal hemoglobin by hydroxyurea, currently approved for treatment of SCD patients in acute vaso-occlusive crisis, is based on that principle (though it has still not been conclusively proven that the therapeutic effect of hydroxyurea is mediated through fetal hemoglobin induction).
The ultimate long-term objective will be to bring new fetal hemoglobin inducers to clinical practice to improve upon drugs such as hydroxyurea, the only FDA-approved drug for treatment of sickle cell disease.
Hydroxyurea is thought to work in part through fetal hemoglobin induction, but does not benefit all patients.

Smoking during pregnancy has been linked to a variety of adverse pregnancy outcomes
Hemoglobin adducts originally were suggested as a biochemical monitor of carcinogen exposure and a measure of genotoxic risk by Ehrenberg, Osterman-Golkar and others [Ehrenberg 1977; Ehrenberg 1992; Osterman-Golkar 1983; Segerback 1978; Tornqvist 1986].
The investigators are aware of a recent report in the literature on the use of cotinine as an indicator of smoke exposure (Haufroid V, 1998) which reviews the effectiveness and practicality of cotinine as a reliable indicator of tobacco smoke exposure.
Globin will be prepared by removing the heme from hemoglobin using acidified acetone conditions.

They concluded that moderate to high levels of fetal hemoglobin (5.4-9.7% to 39.8%) reduced the risk for early onset of dactylics, painful crises, acute chest syndrome, and acute splenic sequestration.
The severity of this condition is quite variable.
I have reviewed the role of some of these factors, and tried to ascertain the clinical importance of each.

Ask A Scientist - Fetal hemoglobin
Scientists think that the higher oxygen affinity of fetal hemoglobin facilitates the transport of oxygen from mother to fetus.
Therefore, it is said to have higher oxygen affinity.
If at a given concentration of oxygen the mother's hemoglobin is more likely to unload oxygen while the fetus's hemoglobin is more likely to bind oxygen, then the oxygen is transferred from the mother to the fetus.

Effect of hydroxyurea on the frequency of painful crises in sickle cell anemia (original 1995 study)
Combination therapy with hydroxyurea and recombinant erythropoietin —rather than treatment with hydroxyurea alone—has been shown to further elevate hemoglobin F levels and to promote the development of HbF-containing F-cells [1].
These invariably lead to so-called painful vaso-occlusive episodes, which are a hallmark of the disease.

The Agency for Health Care Policy and Research (AHCPR) has recommended hemoglobin electrophoresis, isoelectric focusing and high-performance liquid chromatography as accurate methods for newborn screening ( 2).
This association is most likely related to the fact that the mean hemoglobin concentrations and the mean fetal hemoglobin levels differ among the haplotypes ( 15), both which have been shown to be correlated with clinical expression of sickle cell disease ( 10, 16).
Increased levels of fetal hemoglobin (Hb F) are associated with a less severe clinical course of sickle cell disease ( 16).

Fetal hemoglobin (Hb F) is the predominant hemoglobin of the developing fetus.
Thus, in the case of hemoglobin, there are stunning series of changes (called switches) from one type of hemoglobin to another as development progresses from the embryo to the fetus to the newborn, etc.
