CHEST: Femoral vein pulmonary artery catheterization in the intensive care unit
The femoral vein was then punctured percutaneously one finger breadth below the inguinal ligament and approximately 1 cm medical to the femoral pulse with a 21-guage "finder" needle.
Once the vein was localized, an 18-guage needle was placed directly adjacent to the finder needle and via Seldinger technique an 8-French Swan-Ganz catheter introducer (Sterile Concept, Richmond, Va) was placed.
The charts were analyzed looking for ventricular arrhythmias at the time of catheter insertion and evidence of catheter-related septicemia, clinical evidence of deep vein thrombosis and pulmonary embolism, and problems developing at the insertion site such as hemostasis or localized infection.

sBMJ Picture Quiz: Know your anatomy--the femoral triangle
The femoral artery lies at the midinguinal point, which is midway between the pubic symphysis and anterior superior iliac spine and is the inferior extremity of the midclavicular line.
The pubic symphysis is a secondary cartilaginous joint between the bodies of the pubic bones, and the pubic tubercle is a small bony projection on the upper surface of the body of the pubis (fig 2).
Palpate the femoral pulse at the midinguinal point.

The vein is identified by its position relative to the artery, absence of pulsatility, and demonstration of collapsibility by applying gentle pressure.
The main limitations of this study are the few number of patients involved and the possibility of interobserver variation in the estimation of femoral vein diameter with the ultrasound technique used.
Their study populations were older, however, and the technique of measurement of the internal diameter of the femoral vein differs from ours.

 Anatomy
The internal jugular vein exits the jugular foramen of the skull base and courses inferiorly along with the carotid artery and vagus nerve The IJ begins posterior to the carotid artery at the cranium but spirals around the artery and ends up anteriorly at the level of the chest.
Commonly the carotid artery is manually displaced medially and a needle puncture is made midway between the angle of the mandible and superior margin of the clavicle directed towards the ipsilateral nipple.
The down side is that this approach, however, is that it still may incite the same amount of stenosis and thrombosis as the subclavian approach.

In another study of 183 patients, 5.5% of patients had a variant right IJ vein, which did not correlate to the traditional anatomical landmarks (24) And in a third study, 5.5% of 869 patients had an IJ vein that was actually medial, rather than lateral, to the carotid artery (25).
The practitioner should then mark the vein’s location by using a needle hub to indent the skin, both anteriorly and posteriorly to the midpoint of the probe (Video 8).
To cannulate the vein, the catheterization needle should puncture the skin at the superior apex of this triangle while aiming for the patient’s ipsilateral nipple.

This term, historically, has not used by anatomists and has fallen out of favour with most physicians because it has lead to considerable confusion with its accompanying vein, the femoral vein, which if called superficial femoral vein may incorrectly be assumed to be a superficial vein, as opposed to a deep vein.
For a while at this location, (the femoral triangle), it is sometimes referred to as the common femoral, because it has not yet branched.
