 Neonatal Disorders
Phototherapy is the treatment of choice for physiologic jaundice and pathologic jaundice from erythroblastosis fetalis (after the initial exchange transfusion).
Neonatal therapy for hydrops fetalis consists of maintaining ventilation by intubation, oxygenation, and mechanical assistance, when necessary; and removal of excess fluid to relieve severe ascites and respiratory distress.
In severely affected neonates who survive birth, erythroblastosis fetalis usually produces pallor, edema, petechiae, hepatosplenomegaly, grunting respiration's, crackles, poor muscle tone, neurologic unresponsiveness, possible heart murmurs, a bile stained umbilical cord, and yellow or meconium stained amniotic fluid.

This period and, in fact, the next 20 years beyond must be looked upon as the age of treatment-a remarkable period of 40 years in which therapy markedly reduced mortality and morbidity and, then, ultimately almost entirely eliminated the disorder through prophylaxis with anti-D immune globulin.

eMedicine - Hydrops Fetalis : Article by George Cassady, MD
Sex: Sex influences in incidence or outcome of hydrops fetalis are related largely to the cause of the condition.
This condition, ranging from 1 in 500 to 1 in 1500 in a Thai population, has been considered to be a fatal fetal condition (Bart hydrops).
Thus, some health care professionals have questioned the practical and ethical basis of fetal and neonatal treatment.

In other fetuses, a clinical suspicion of hydrops fetalis may exist because of a previous family history of a similarly affected baby or because ultrasonography is performed to evaluate polyhydramnios.
False Positives/Negatives: A few conditions mimic full-blown hydrops fetalis, but individual components of hydrops fetalis can be seen in other conditions, even as normal variants.
The condition is uniformly fatal and associated with a significant risk of maternal morbidity.

When erythroblastosis does occur, whole-body transfusions of the affected baby can effectively flush out the offending and sometimes lethal antibodies.
Fortunately, thanks to the work of medical investigators, preeminent among whom was Dr. Bruce Chown, the disease can be prevented.
A child diagnosed as having erythroblastosis fetalis can be affected in many ways, from mild anemia to jaundice and prematurity.

Reacting to the anemia, the fetal bone marrow may release immature RBCs, or erythroblasts, into the fetal peripheral circulation, causing erythroblastosis fetalis.
The resulting anemia may be so profound that the fetus may die in utero.
High-resolution spectrophotometry is useful in the antepartum assessment of erythroblastosis fetalis (see Table 252-2).

Erythroblastosis fetalis - Wikipedia, the free encyclopedia
Erythroblastosis fetalis, also known as hemolytic disease of the newborn is a condition that develops in a fetus when antibodies produced by the mother attack the fetus's red blood cells.
Symptoms include yellowish skin and eye discoloration within 24 hours after birth, a severe form of Hydrops Fetalis ( pallor, an enlarged liver and/or spleen, generalized swelling, respiratory distress, petechiae, purpura, death in uterus or shortly after birth).
Problems include Rh Incompatibility, ABO incompatibility or other blood group incompatibilities such as c, E, Kell(K).

 Erythroblastosis fetalis
This condition is worse in successive at-risk pregnancies.
Erythroblastosis fetalis is the condition in which a pregnant women produces an antibody (IgG) to the red blood cells of the fetus she is carrying, which causes the destruction of her fetus' red blood cells.
Symptoms include lots of erythroblast cells which are circulating in the blood instead of being within bone marrow, liver and spleen enlargement, and edema (tissue swelling due to excessive fluid).

When severe, the anemia can result in extramedullary hematopoiesis, secondary organ dysfunction, heart failure, hydrops, and death.
The name erythroblastosis refers to the presence of immature erythrocytes in the peripheral blood from accelerated hematopoiesis.
Can occur when the fetus inherits a paternal blood group antigen lacking in the mother.
