Postgraduate Medicine: A practical workup for eosinophilia
Eosinophilia may be associated with respiratory symptoms, often asthmatic in nature, together with pulmonary infiltrates that are detectable on chest films (12).
Striking eosinophilia can occur in patients who have eosinophilic gastroenteritis, often accompanied by a history of atopy, and may cause abdominal pain without specific features or diarrhea and intestinal obstruction.
However, a more practical approach is to consider the degree of eosinophilia along with the clinical setting in which it was discovered (2,6,7).

eMedicine - Eosinophilia-Myalgia Syndrome : Article by Nicole Quartarolo, MD
Serotonin overproduction by carcinoid tumors is associated with myalgias and arthralgias in addition to sclerodermalike skin changes.
This phase lasts weeks to months and is followed by a chronic phase characterized by sclerodermoid skin changes, neuropathy, neurocognitive deficits, continued myalgia, and muscle cramps.
This condition usually begins in the proximal muscle groups, such as those in the shoulders, the buttocks, and the thighs; then, myalgia becomes incapacitating.

Mayeno and colleagues may be well on their way toward affirming that our scientific world continues to shrink when neither time nor distance deters investigators who have the will to persevere.
The investigators in Japan did not identify other likely candidates in their studies.

While not all patients may exhibit eosinophilia, many patients go on to experience peripheral edema -- both pitting and nonpitting, as well as features of peripheral neuropathy, myocarditis, pneumonitis, fasciitis, or a scleroderma-like syndrome.
Therefore, any new symptom or condition observed in EMS patients should be evaluated for potential relationship to the progression of EMS.
EMS was originally described in 1989 as a symptom complex associated with the ingestion of LT. Although the majority of the individuals originally described developed significant eosinophilia (>1000/mm) and debilitating muscle pain, it is now recognized that the syndrome may encompass a wide range of symptoms that may change over time.

Over time many other symptoms developed in patients that led in some cases to death and in many other cases to serious long term disability.
The disease caused by this toxic product was called eosinophilia myalgia syndrome or EMS, because the initial symptoms were elevated numbers of blood cells called eosinophils and myalgia (muscle pain).
Based on these points, we conclude that it is highly likely that genetic engineering was the determining factor in generating this toxin.
