A 'Whipple' is a possible surgery that is tried sometimes with this cancer.

Stomach cancer and gastric or duodenal ulcer disease
This increase in cancer incidence was not found in a subgroup of 8,646 patients with pre-pyloric gastric ulcers, however.
The cohort consisted of 29,287 patients with gastric ulcer only, 24,456 with duodenal ulcers and 4.193 with both (analyzed separately).
The incidence of cancer was then compared to that of the Swedish population at large.

Duodenal polyps are adenomas and appear to have some malignant risk as a 5% lifetime occurrence of duodenal cancer is observed in persons with FAP.
The average age of colon cancer is 39; 7% of untreated persons already will have developed colon cancer by age 21.
The lifetime risk of colon cancer for both men and women in the U.S. is approximately 6%.

Cell Proliferation of the Duodenal Mucosa in Patients Affected by Familial Adenomatous Polyposis.
Standard forward viewing endoscopy was used, whereas in clinical practice side viewing endoscopy is recommended since duodenal polyposis in FAP is more severe in the periampullary region[1] and this is likely to be missed with standard endoscopy.
Our other concern relates to the quoted cancer incidence, which we feel must be biased.

Duodenal acidity is probably of particular concern in patients who have undergone a duodenum-preserving pancreatic head resection, since, in this anatomic situation, pancreatic juice transits directly via the jejunal loop, bypassing the duodenum.
Diabetes mellitus, other medical conditions and familial history of cancer as risk factors for pancreatic cancer.
Patients undergoing a Whipple procedure or side-to-side pancreaticojejunostomy are probably less critically affected because secretions transit, at least in part, via the papilla.

My surgeon had told me that he had another female patient older than me with the same cancer and she was cancer free 3 years after her surgery.
I live in Texas and had tried to get in to MD Anderson, but they only have one dr. who treats this kind of cancer and it is a first come, first serve basis, and I didn't start the process of getting in there until a month after my surgery.
Because it is so rare, they don't want to check for it - I remember my dr. saying he had another patient with a tumor in her small intestine, but "it is so rare".

Editor's Comment: - Both gastric ulcers and stomach cancer are associated with infection by Helicobacter pylori, so the findings regarding stomach ulcers are to be expected, but the results for duodenal ulcers are difficult to understand.
As reported in the July 25, 1996 issue of the New England Journal of Medicine, the incidence of stomach cancer in those with gastric ulcers peaked in the first 3 years and leveled off at a value which was about 80% greater than for the general Swedish population.

Section 5 Chapter 14 Hereditary Colorectal Cancer and Polyposis Syndromes
Thus, the decision between prophylactic surgery and surveillance for gene-positive unaffected patients is based on many factors, including the penetrance of disease in a family, the age of cancer onset in family members, functional and quality-of-life considerations, and the likelihood of patient compliance with surveillance.
Foci of carcinoma in situ and invasive carcinoma may be found within larger polyps, and the incidence of invasive cancer is proportional to the extent of polyposis.
The main considerations to be taken into account are the risk of rectal cancer development if the rectum is left in situ and the differences in functional outcome (and associated quality of life) between procedures.
