Directly Observed Therapy Being Considered for HIV Infection
Directly Observed Therapy Being Considered for HIV Infection
Clinical trials should be designed with adequate follow-up to compare retention on therapy, virologic and immunologic outcomes, evaluation of the development of drug resistance, and cost-effectiveness analysis, the authors conclude.
The primary reason for considering DAART is the large role that inadequate medication adherence plays in virologic failure, Dr. Lucas' group notes in their review published in the November issue of AIDS Patient Care and STDs.

Directly Observed Therapy Effective in Difficult-to-Treat HIV-Positive IV Drug Users
Methadone and antiretroviral drugs were dispensed daily and administered to patients as part of the directly observed therapy.
Triple therapy with nevirapine (NVP), didanosine (ddI), and stavudine (3TC) was given to most once-daily patients.
The majority of patients in the study achieved viral suppression to undetectable levels and the study data indicate DOT may be an appropriate method of combining addiction treatment with HIV therapy in these difficult-to-treat patients, he said.

 TB Policies: Directly observed therapy
DOT is a method where every dose of anti-tuberculosis medications taken by the patient is directly observed and supervised by a health care worker or other responsible individual.
An individual who is being cared for by a non -NYCDOH physician may also receive DOT at a NYCDOH Chest Clinic, in which case the Physician-in-Charge must review the individual's medical regimen.
If an individual is not being treated under a program of DOT, the physician must document the reason in the individual's clinic medical record.

American Thoracic Society Journal news tips for September 2004 (first issue)
Directly observed therapy was defined as ingestion of anti-tuberculosis medications under the direct supervision of a health care worker.
When investigators compared tuberculosis (TB) patients who participated in directly observed therapy to patients with risk factors for non-adherence who performed self-administered therapy, they found that cure occurred in almost 98 percent of the patients treated by directly observed therapy as contrasted to a rate of slightly over 80 percent for the self-medicated patients.
The authors said that 44 percent of patients who receive self-administered therapy had risk factors and should have been assigned to directly observed therapy instead.

Doubts About DOT: Antiretroviral Therapy for Resource-Poor Countries
DOTS is a multifaceted approach to TB therapy, including observed pill-taking, community and local government commitment, case detection by sputum smear microscopy, stable drug supply, and standardized systems for case follow-up.
The call for directly observed HIV therapy is rooted in experience with TB treatment.
While the public health advantages of directly observed HIV therapy are in question, the impact on individual freedoms is not.

 TB News: Current News Article
In response, Heymann, of the Harvard School of Public Health, and others note that directly observed therapy is not necessary for TB-incidence reduction, as evidenced by the decline in U.S. TB cases between 1953 and 1985, when the practice was not used.
With the development of effective antimicrobial therapy, sanatoria closed and the tuberculosis physician acquired a new body of knowledge and developed skills in pulmonary function testing, bronchoscopy, and, later, polysomnography.
Furthermore, the authors contend that directly observed treatment does not often use confinement to motivate patients to comply; rather, they point out that incentives and enablers are more commonly used.

UCSF HIV experts question observed therapy in poor nations
The editorial notes that DOT for tuberculosis therapy became a public health imperative to prevent emergence of drug resistance strains of TB and that the possibility of creating drug resistant strains of HIV is cited as a rationale for DOT for HIV.
The authors observe that there is no evidence yet that DOT is superior to self-administered therapy for reducing transmission through treatment, and if it were proved to be so, that the discussion of HIV DOT should not be limited to resource poor countries.
A team of HIV researchers at UCSF recommend against establishing directly observed therapy (DOT) as the model for the provision of HIV medications in resource poor countries.

HIV Report July 2001 - Directly Observed Therapy
While DOT may ultimately play a part in the treatment of HIV, it seems likely that its role will be considerably more focused than the cornerstone position it assumes in TB treatment.
Adherence was 91% with the self-administered drugs and 86% with the directly observed drugs.
A considerable question left unanswered at the workshop was the long-term role of DOT in HIV therapy.

 Making DOT Work
DOT can take place in the office, clinic, or in the community and can be used alone or with other measures.
Success in DOT depends on an outreach worker's ability to problem solve, facilitate and empathize with his or her client.
Often the relationship that develops between the health care worker and the patient in the course of providing DOT contributes greatly to its success.

Furthermore, it helps to recall that many earlier studies of combination therapies excluded patients like those studied by Jayaweera because of the likelihood of treatment non-adherence; efforts like this study should be applauded for their attempts to evaluate and improve the care for some of the most difficult-to-treat persons in our communities.

Direct observation of people taking their antituberculous drugs is widely advocated and forms part of the World Health Organization's 'directly observed therapy, short course' (DOTS) strategy.
The objective of this article produced by the Cochrane Review is to compare policies of directly observed therapy with self treatment at home in people requiring treatment for clinically active tuberculosis, or requiring medication for prevention of active disease, on cure and/or treatment completion.
The article concludes that the effects of direct observation on cure or treatment completion were similar to those of self-administered treatment.
