The authors conclude that desmopressin administration is associated with a significant rise of ACTH and cortisol levels in majority of the patients with Cushing’s disease and there was good (non complete) concordance between the response to desmopressin test and surgical outcome.
Plasma ACTH and serum cortisol were measured using commercially available immunoassays and an increase of at least 30 percent and 20 percent above the baseline of ACTH and cortisol concentrations respectively was used as a criteria to classify it as a positive response.
During follow-up, 3 patients experienced a recurrence of disease 24, 38 and 54 months after surgery and all these patients had persistence of ACTH response to desmopressin after surgery.
