If you will be receiving a general anesthetic during surgery, your doctor will give you the medicine and closely follow your progress.
In deciding to use a medicine, the risks of using the medicine must be weighed against the good it will do.
Your doctor will take your age into account when deciding on the right amount of desflurane for you.

Economic value of Desflurane in comparison to Isoflurane in Low Flow Anesthesia
Patients were tracheal intubated and then in 20 patients the anesthesia was performed using Desflurane and oxygen/nitrous oxide and in 20 patients anesthesia was performed using Isoflurane and oxygen/nitrous oxide at about 1.3 MAC in both groups.
This is no real disadvantage, since fresh gas flows of more than 2 l/min have no clinical advantage in Desflurane use.
Using Desflurane in this fresh gas flows deems not clinically necessary.

Review Of Currently Used Inhalation Anesthetics
Enflurane, isoflurane, desflurane, and sevoflurane produce skeletal muscle relaxation that is about twice as high than that associated with halothane.
Nitrous oxide does not relax skeletal muscle and may, especially in combination with opioids cause muscle rigidity.
Volatile anesthetics do not only potentate the action of neuromuscular blocking agents but also have muscle relaxants properties of their own.

 Drug Discovery & Development
Previous research has shown that desflurane was neuroprotective during deep hypothermic circulatory arrest (DHCA), one of the modalities used during infant cardiac surgery.
Loepke says that, "If results in humans parallel our results in animal studies, desflurane or similar anesthetics could contribute to clinical improvements for children with heart conditions." Research continues to determine how desflurane actually protects the brain from ischemia in the surgical setting.
According to Andreas Loepke, MD, PhD, a pediatric anesthesiologist at The Children's Hospital and lead author of the study published in the December 2002 issue of Anesthesiology, the study's results show that desflurane seems to be protective during circulatory arrest as well as during low-flow cardiopulmonary bypass surgery.

Desflurane is one of a family of halogenated methylethylethers which is administered by inhalation producing a dose-related, reversible loss of consciousness and of pain sensations, suppression of voluntary motor activity, modification of autonomic reflexes and sedation of respiration and the cardiovascular system.
If added relaxation is required, supplemental doses of muscle relaxants may be used.
Nausea and vomiting have been observed in the postoperative period, common sequelae of surgery and general anaesthesia, which may be due to inhalational anaesthetic, other agents administered intraoperatively or post-operatively and to the patient's response to the surgical procedure.

In a finding that may ultimately benefit children undergoing heart surgery as infants, researchers showed that a particular anesthetic medication improved neurological outcomes in animals that underwent cardiopulmonary bypass (CPB) under surgical conditions.

The researchers remain uncertain about the process by which desflurane improved neurologic outcomes in their study, although they speculate that it may decrease the amount and effects of deleterious amino acids in the brain.

ASA: Desflurane at High Concentrations Appears to Have More Adverse Events Than Sevoflurane
Shankar pointed out that confounding factors -- such as the use of nitrous oxide with both drugs and a higher use of opioids with desflurane -- might have contributed to the earlier finding.
He concluded that desflurane irritates the airway enough so that its use could be a problem in some patients, especially at higher concentrations.
None of the sevoflurane patients had any of those reactions, Dr. Shankar said.

However, in contrast to desflurane, at concentrations exceeding 1 MAC sevoflurane has little or no respiratory irritant properties and may be used to rapidly induce anesthesia.
