Topic: Dermatitis herpetiformis

 Dermatitis Herpetiformis
Dermatitis herpetiformis [DH] is associated with gluten-sensitive enteropathy [celiac disease] and is characterized by a chronic, intensely pruritic papulovesicular rash symmetrically distributed over exterior surfaces of the body, the trunk, buttocks, scalp, and neck.
Administration of dapsone or other sulfones often relieves the skin itching associated with the skin lesions of dermatitis herpetiformis.
There is strong evidence that the changes in the intestinal mucosa and the immunologic findings in the majority of patients are identical to those found in celiac disease.

Dermatitis Herpetiformis; Gluten; Course Work; Abstracts
Jejunal villous atrophy was present in 78% of our dermatitis herpetiformis patients, and a single jejunal biopsy was as effective at detecting this as the multiple biopsy technique.
Dermatitis herpetiformis bodies were not found in normal human skin.
Abstract Intestinal permeability was investigated in patients with coeliac disease and dermatitis herpetiformis by a 51Chromium-labelled ethylenediaminetetraacetate (51Cr-EDTA) absorption test and the results correlated with histomorphometric analysis and intraepithelial lymphocyte counts of jejunal biopsies.

Introduction to Celiac Disease, Dermatitis Herpetiformis, and Wheat Allergy
Symptoms of wheat allergy could be eczema (different from dermatitis herpetiformis), sneezing, increased acne, or if you have a very serious allergy to wheat, you might have an anaphylactic reaction in which your throat would swell up to the point where you could no longer breath.
In this form of the disease, lesions that look like a herpes sore ("formis" means "in the form of" and "herpeti" refers to the similarity to herpes) appear on the skin, usually in very symmetrical ways: on both hands, both arms, head, elbows, knees, buttocks, etc. They are often extremely itchy.

When a biopsy is performed on uninvolved skin (2—6 cm distant from a lesion), there is generally less intense deposition of IgA and occasional biopsy results may be negative.
Because of the intense pruritus, the small grouped papulovesicles are rapidly scratched, leaving only excoriated papules clinically.
Approximately 10%—15% of first-degree relatives of patients with dermatitis herpetiformis have celiac disease or dermatitis herpetiformis.

This pattern is found in 40% of individuals with Dermatitis Herpetiformis and a portion of first degree relatives of celiacs who have no gastrointestinal symptomatology.
It is important to keep in mind that intestinal biopsies are most commonly obtained endoscopically from the duodenum and therefore do not provide information as to the extent of disease along the length of jejunum.
This pattern is classically found in individuals with celiac disease who have gastrointestinal symptoms, but has also been reported in asymptomatic celiac relatives and individuals with Dermatitis Herpetiformis.

 Celiac Disease
Dermatitis herpetiformis (DH) is a severe itchy, blistering skin disease caused by gluten intolerance.
DH is related to celiac disease because both are autoimmune disorders caused by gluten intolerance, but they are separate diseases.
The connection between celiac and these diseases may be genetic.

Dermatitis Herpetiformis and Celiac Disease are in fact the same disease, and prof Jarmo Visakorpi, Tampere, Finland has suggested the name "Skin Celiac Disease".
Nearly all patients with DH do have changes in the bowel mucosa identical to celiac disease, and the skin lesions are almost always cured by a strict glutenfree diet.
The itching caused by Dermatitis Herpetiformis can be severe, and the skin changes are partly caused by scratching.
