Department of Pediatrics Robert W. Wilmott, M.D. ,IMMUNO Professor & Chair Saint Louis University School of Medicine
The purpose of the Section of Hemostasis and Thrombosis in the Division of Hematology/Oncology is to provide multidisciplinary care and to facilitate clinical research for patients with the chronic disorders of inherited coagulopathies, thrombophilia, and HIV exposure/infection.
A team approach utilizing a specialized staff of nurses, social workers, physical therapists, and physicians work with patients and their families to promote independence, understanding and prevention of long term disabilities.
Special services include a Comprehensive Hemophilia Clinic which provides the multidisciplinary care for patients with chronic disorders of inherited coagulopathies, and the HIV/AIDS clinic which provides comprehensive care including pediatrician/hematologist, immunologist and nursing coordination for the individual patient’s care.

Hematopathology Fellowship, Department of Laboratory Medicine at Yale
The program takes advantage of the rich environment in both anatomic and clinical pathology at these institutions with a special emphasis on integrating all aspects of clinical practice, teaching and research with the other academic activities in Pathology and Laboratory Medicine at Yale.
The basic "how-to" of procedures is also taught during this initial set of rotations - this includes, for example, doing bone marrow aspirates and biopsies on the floor and interviewing patients with coagulopathies (both during the heme/coag and flow/molecular rotations).
In each rotation, the role of the pathologist as consultant is the focus of learning - trainees communicate results of studies directly to other clinicians, albeit during this initial period of time that communication is often carried out in the presence of the pathology attending.

Guidelines for the Management of Bleeding
Most coagulopathies of primary or secondary hemostasis are acquired due to an underlying systemic disorder.
Perform these tests in the absence of anticoagulant therapy but when bleeding suggests a coagulopathy.
Useful when anticoagulant therapy has been ruled out and when bleeding suggests a coagulopathy.

 Thoracic surgery
Patients who have blood-clotting problems (coagulopathies), and who have had previous standard thoracic surgery may not be good candidates for video-assisted thoracic surgery (VATS).
Blunt chest trauma, reflux esophagitis, esophageal cancer, lung transplantation, lung cancer, and emphysema are just a few of the many clinical indications for thoracic surgery.
Because VATS requires the collapse of one lung, potential patients should have adequate respiratory function to maintain oxygenation during the procedure.

 OSTEONECROSIS
One of the major aims of the Center for Osteonecrosis Research and education is to conduct basic science experiments pertaining to the pathogenesis of this disease process.
As recent reports in the medical literature have provided evidence that patients with osteonecrosis may have defects in their blood clotting system, we currently screen all of our osteonecrosis patients for coagulopathies.
Larger numbers of patients and matched control subjects are needed to define the association between coagulopathies and osteonecrosis.

The use of recombinant factor VIIa may be a valuable therapeutic option in children with severe acquired coagulopathies since it provides a good hemostatic efficacy and probably few adverse side-effects.
Besides this well recognized therapy rFVIIa is used to control bleedings caused by a variety of diseases like thrombocytopathies, liver disease, during liver transplantation or cardiac surgery.
In contrast to rising experience obtained by ongoing clinical trials in adults, poor data exit for the use of rFVIIa in acquired coagulopathies in childhood.

It is best to determine the underlying cause, and treat the specific problem, rather than attempting only symptomatic therapy.
Coagulopathies or bleeding disorders should be considered, especially if there is evidence of bleeding from other body sites as well.
The most common causes are usually diseases or disorders of the upper gastrointestinal tract, although in some cases, clotting disorders (coagulopathies) may result in bleeding even though the gastrointestinal tract is essentially healthy.

Few human studies of factor VIIa exist, and most of those that have been done included only patients with preexisting coagulopathies.
Factor VIIa has been used as a treatment for hemophilia for years; it has been shown to be effective in patients with other coagulopathies.
Most evidence to support the use of factor VIIa in trauma patients is anecdotal, consisting of testimonials and case reports.

Medical Support of the US Army in Vietnam
To see a patient suddenly begin to bleed profusely from every orifice and wound just as heroic surgery appeared to be successful was a dramatic experience.
Fortunately the number of patients suffering from coagulation problems was small, but the threat after massive, transfusions and surgery was ever present.
Since coagulopathies usually occur in combinations and rarely in the pure form, study under field conditions is almost impossible.

The most common vitamin K-responsive coagulopathy is anticoagulant rodenticide intoxication, although vitamin K deficiency may also occur with malnutrition and intestinal malabsorption, extended oral antibiotic therapy, and hepatic disease.
Abnormalities of coagulation may be acquired or inherited, however, acquired causes of abnormal coagulation constitute the majority of cases seen in clinical practice.

Perioperative or postoperative coagulopathies can be caused by the surgical procedure itself, by the pre-existing coagulopathy of the patient, accompanying pathologies, administered medications, hypothermia and dilutional effects.
Instead of being treated with medical hemostasis, stress and coagulopathic conditions caused by parenchymatous organ injuries due to blunt traumas may result in unnecessary surgical interventions.
In patients undergoing prolonged surgical procedures, in those with trauma [1] and hemorrhage, bleeding resulting from disturbances of coagulation cascade and factor deficiencies has been observed.

Dental management considerations for the patient with an acquired coagulopathy.
Current teaching suggests that many patients are at risk for prolonged bleeding during and following invasive dental procedures, due to an acquired coagulopathy from systemic disease and/or from medications.
Making changes in anticoagulant drug regimens are often unwarranted and/or expensive, and can put patients at far greater risk for morbidity and mortality than the unlikely outcome of postoperative bleeding.

 Hematologic Complications of HIV Infection
This review explores the causes of isolated or trilineage cytopenias and coagulopathies; the utility of bone marrow biopsy examination; and the role of colony-stimulating factors as therapeutic agents in patients with HIV disease.
Hematologic complications of HIV infection are common and challenge physicians treating patients at all stages of HIV disease [1,2].
Ineffective hematopoiesis, infiltrative diseases of the bone marrow, nutritional deficiencies, peripheral destruction of blood cells secondary to splenomegaly or immune dysregulation, and drug effects all contribute to the variety of hematologic abnormalities seen in these patients.

Inherited coagulopathies are among the major causes of thromboembolism; these disorders are thought to be responsible for approximately half of all DVT.
In addition, many patients with an identifiable acquired or transient provoking risk factor should also be screened for inherited coagulopathies since 50% of the DVT in patients with thrombophilia are provoked by a recognized risk factor.
The risk of maternal thrombosis is further increased 8 fold in patients with a hereditary coagulopathy compared to women without such an abnor-mality.
