Thomas Willis - Wikipedia, the free encyclopedia
Willis was the first natural philosopher to use the term " reflex action " to describe elemental acts of the nervous system.
Willis astonishing insightful forays into the anatomy and physiology of the brain were extremely influential.
His observation of the connection of the eighth pair with the slender nerve which issues from the beginning of the spinal cord is known to all.

In regard to the findings of Willis and his circle, an important point to emphasize is that much of their ‘new’ cerebral anatomy would have been impossible from inspection of brains ravaged by the usual post-mortem disintegration.
Willis also lived here when he was carrying out the programme of brain dissection with his group in the early 1660s.
‘Thomas Willis practically refounded the anatomy and physiology of the brain and nerves....

However, Willis contributed to the acceptance of quinine and reintroduced the colchicine therapy of gout, and he invented several medicines.
Pharmaceutice rationalis, with case histories, post mortems, and therapies, attempted to establish pharmacology as a science based on anatomy, morbid anatomy, and chemical experimentation.
This is a typical example of Willis's ability to make application of isolated, but important facts.

 About Circle of Willis
After taking an anatomy course in which we were given the opportunity to dissect human cadavers I came across the Circle of Willis while studying the vasculature of the brain.
A natural communication between two vessels; may be direct or by means of connecting channels.
I was immediately captivated by the peculiar shape of this structure and thought that it would make an interesting necklace.

However further studies need to be done to assess the reliability of this technique for assessment of pathological changes of the arterial segments.
Observer agreement of the two techniques is fair to perfect except for the PCOM segment where the agreement is only slight, stressing the point that the non-contrast enhanced 3D TOF MRA technique is not reliable for the assessment of the PCOM segment.
Failure to identify the communicating arteries could be related to non-patency (atresia, thrombosis or vessel spasm); an absence of significant flow in the baseline conditions i.e.

There is therefore a great importance on the understanding of factors that increase the risk of stroke and hence how blood is distributed throughout the brain.
The long term goal of the project involves the development of a clinical diagnostic tool for automatically recreating a 3D model of an individual’s cerebral vasculature, from an MRI scan and using CFD to predict the likelihood of stroke in the short term, or the risk associated with various surgical procedures such as carotid endarterectomy.
The current research is focused on a ring-like arterial structure located in the base of the brain, known as the Circle of Willis.

 Abstract
Although Dr.Thomas Willis is best remembered for the accurate description of arterial anastomosis at the base of the brain, his contribution to neuroanatomy, physiology and medical science in general is vast, and several diseases bear his name.
With the advances in microneurosurgery and the ability to tackle diseases of the intracranial arteries at the base of the brain (often referred to as the Circle of Willis) surgically more effectively, accurate knowledge of the intracranial vascular anatomy is increasingly important.

 Brain Circulation
Although flow is usually from internal to external carotid circulations, blood can flow in either direction through this vessel, so it may serve as a colateral to the brain or to the face.
As the angles of head and neck and as the rotational positions of the head on the neck all change, the internal carotid and/or vertebral of one side may be partly occluded.
Portions of the circle may also be involved in thrombosis, clots.

 Circle of Willis
Damage to the circle itself is often clinically insignificant, because blood can flow in either direction around it.
IRCLE OF The system of arteries which conduct blood to the brain, the Basilar and the Internal Carotids, are connected in a rather special way.
Because this loop is supplied by three vessels, in an arrangement which provides collateral flow, damage to any one will not compromise the blood supply to the brain.

Lessons Learned from a Case Report of a Traumatic Intracranial Aneurysm
A case of a ventral internal carotid artery (ICA) aneurysm located within the circle of Willis is presented to highlight the importance of keeping in mind that this location can be a site of such aneurysms.
The aneurysm was diagnosed approximately two weeks after a head injury when the patient had a delayed neurological deficit due to vasospasm.
Unlike the reports in the literature, the traumatic aneurysm in the present case was located within the circle of Willis- at the branch point of the posterior communicating artery.

Analysis of the collateral pathways in relation to carotid test clamping in awake patients may elicit the indication of selective shunting.
The rate of intraoperative cerebral ischemia was significantly higher in patients with poor collateral circulation defined by the anatomy of the circle of Willis.
Mandatory shunt was significantly higher in patients with absence of collaterals (p = 0.00).

One is the persistence of the fetal pattern of trifurcation in which all 3 of the ipsilateral major cerebral arteries arise from the internal carotid while the posterior cerebral segment of the circle continues, as in the early fetus, to be a small, threadlike, posterior communicating type of vessel [B].
The opposite, anterior cerebral segment of the circle, however, may be missing, in which case the contralateral internal carotid simply continues as the middle cerebral artery or may bifurcate into the posterior cerebral artery and the middle cerebral artery [C].
There are 2 relatively common, variations from the normal pattern of branching of internal carotid artery.

 Bambooweb: Aneurysm
Aneurysms most commonly occur in the arteries at the base of the brain (the circle of Willis) and in the aorta (the main artery coming out of the heart) - this is an aortic aneurysm.
While most aneurysms occur in an isolated form, berry aneurysms of the anterior communicating artery of the circle of Willis is associated with autosomal dominant polycystic kidney disease (ADPKD).
The layer of the artery that is in direct contect with the flow of blood is the tunica intima, commonly called the intima.

 Blood Supply
The anterior cerebral artery supplies the frontal lobes and medial aspects of the parietal and occipital lobes.
The internal carotid artery supplies the brain itself.
It is commonly divided into its intracranial part which consists of the carotid siphon which traverses the base of the skull (cranium) and its extracranial part which branches into the middle cerebral, anterior cerebral and posterior cerebral arteries to form the Circle of Willis.

The area of intracerebral haemorrhage often causes an increase in intracranial pressure.
Stroke Stroke (cerebrovascular accident-CVA) can be defined as a focal disturbance of cerebral function lasting greater than 24 hours (Less than 24 hours is a transient ischaemic attack).
The cause of these aneurysms is not known although hypertension is not thought to be important in their formation.

Research Findings Register: summary number 458
The general fluid blood mass flux is determied primarily by the peripheral resistance of each of the major efferent arteries, viz, posterior, middle and anterior.
In order to properly determine the flow dynamics new and innovative outlet boundarty conditions needed to be derived which correctly model the peripheral resistance
In terms of worst case scenario the absence of the ipsilateral posterior communicating artery could prove fatal in that this provides the largest fall in blood mass flux through the CIrcle of Willis.

 NPEL: Vascular Disease
The anterior and posterior blood vessels have a significant collateral relationship in the form of the circle of Willis.
Second most common cause is the rupture of a Berry (saccular) aneurysm.
On the other hand, because of the constancy of the anterior communicating artery, it is rare to develop a vascular insult in the distribution of the anterior cerebral artery due to occlusion of the ipsilateral carotid artery.

THE MERCK MANUAL--SECOND HOME EDITION, Introduction in Ch. 86, Stroke
If the diameter of this artery is large, the circle of Willis can redistribute blood to the whole brain when one or even two of the large arteries are blocked.
The branches carry blood to all parts of the brain.
These branches empty into a circle of other arteries (circle of Willis), which thus forms a connection between the vertebral and internal carotid arteries.

 CNS Pathology
Such aneurysms are "congenital" in the sense that the defect in the arterial wall is present from birth, but the actual aneurysm takes years to develop, so that rupture is most likely to occur in young to middle age adults.
The circle of Willis has been dissected, and three berry aneurysms are seen.
Multiple aneurysms are seen in about 20-30% of cases of berry aneurysm.
