Topic: Chordee

 Post chordee operation patient
Chordee is usually only recognized when the person is having an erection and so it is frequently not diagnosed untill the parents bring the issue up to the pediatrician.
Chordee surgery is quite complex, as the urethra may be too short and their may be parts of the penis that do not stretch.
I've noticed that since the surgery that my erections don't seem to be as hard and also my ejaculate doesn't have the force it used to, often times just dripping out.

 Urology at MUC-Hypospadias
Bending of the penis on erection may be associated with this and is known as chordee.
When the urethral opening is just slightly out of position and when there is no chordee, surgical correction may not be necessary, although in occasional patients there is a significant cosmetic reason for repair.
Moderate hypospadias with some chordee may require a more extensive operation such as a Mustarde procedure which utilizes a tubularized flap of penile shaft skin.

 Chordee
The goals of surgery are to improve the appearance of the penis for psychological reasons, to construct an organ that allows the patient to void in a standing position, and to produce a sexually adequate organ.
If residual chordee exists and the penis is still not straight, another surgery may be needed to correct the condition.
A chordee may be surgically repaired anytime after six months of age.

When in erection it is considered a significant ventral curvature of the penis (chordee) a deflection angle of the shaft of the penis greater than 20 degrees (3).
Other significant causes of the ventral curvature began to be better defined with the introduction of the artificial erection technique (3).
In order to avoid any accidental lesion to the urethra, next to the meatus, we recommend injection of normal saline using a fine needle (insulin needle) to achieve separation of adherent skin from the urethra.

The nature of the chordee and the location of the meatus may cause painful erections or infertility.
Repair of the chordee is also essential to allow satisfactory sexual function in adulthood.
However, surgical repair is advisable if the urethral opening is in a position which will require the boy to sit down to void, a devastating psychological problem.

 urologyweb Hypospadias
After correction of chordee, or in those boys with distal hypospadias that do not have chordee, the neo-urethra is completed to the tip of the penis, either through the use of existing redundant penile skin, usually as a flap, or with skin taken from another part of the body, as a graft.
While multiple techniques have been described and are available for the repair of hypospadias, marked refinements in surgical techniques, the use of microsurgical instrumentation and fine suture material, and optical magnification have allowed the pediatric urologist to perform the vast majority of hypospadias repairs in a single stage.
Middle and proximal hypospadiac defects are often associated with significant chordee (bending on erection).

 UrologyHealth.org - Epispadias
The primary goals of treatment of epispadias are to:  maximize penile length and function by correcting dorsal bend and chordee ; and create functionality and cosmetically acceptable external genitalia with as few surgical procedures as possible.
The penis may be attached to the pelvic bones, which are widely separated, resulting in a penis that is pulled back toward the body.
Both techniques provide a straight urethra positioned on the underside of the penis, correction of chordee and an acceptable cosmetic result.

He said that he did not see two holes and that it might be a fold or crease in the skin due to his chordee.
I feel he has both but this doctor was positive I was wrong.
I explained to the second Dr. that I can clearly see two holes at the top of his penis I thought that was called hypospadias.

Hypospadias DoctorNDTV: Health Information on Hypospadias
The penis may be curved downward (called chordee) and this is readily apparent during erection.
In most techniques, the skin on the penis or the redundant foreskin is used to create the tube from the existing opening to the tip.
Recently, more and more surgeons are correcting hypospadias by one-stage techniques that reduce hospitalisation and give better cosmetic results in the long-term.

This was also done in patients who have distal chordee.
An objective assessment of this may be performed for evaluating the results of hypospadias surgery after hypospadias repair by HOSE2 or uroflowmetry.3 Uroflowmetry is an easy way of assessing urinary stream and is available in many centres and this may be an important area for future research.
A retrospective observational study was therefore conducted in the department of Paediatric Surgery, National Institute of Child Health Karachi, from December 2003 to December 2004, to evaluate the factors which may influence the results of surgery after hypospadias repair.

Hypospadias Surgery: A High Complication Rate & Satisfied Patients
Chordee was categorized as mild (visible only on erection), moderate (demonstrable without erection), and severe (bent penis bringing tip of glans onto ventral surface).
Mobilising urethra relieves chordee in most of the instances and no further procedure is required to correct chordee.
Six patients had mild chordee barely visible only on erection.

 Chordee
I have not discussed with a doctor but believe that chordee is what I have.
My question is can I still get it corrected and what all does a procedure like that entail and on average what is the cost of an operation like that and would my penis be normal or close to normal.
It may be possible that a urological procedure can correct a chordee.

"Chordee", a bending of the penis on erection, sometimes occurs with hypospadias.
The surgical procedure for hypospadias correction and the surgical recovery time will vary depending upon the severity of the problem.
Because hypospadias involves a malformation of the skin in this area, it may cause abnormal erections (called chordees) and sexual problems in adulthood.

Tubularized, Incised Plate Urethroplasty in Hypospadias Repair
The current concept of understanding of chordee and emphasis upon preserving the urethral plate have brought hypospadias surgery over the last decade near its ultimate goal of a reconstructed penis that is functional and cosmetically normal.
Many techniques have evolved during the past 150 years to address chordee and construct a neourethra.
A subdartos flap was created to cover the neourethra and pressure dressing applied with bactracin gauze for 7 days.

Chordee, ventral curvature of the penis, is frequently seen with hypospadias but sometimes is not recognized until an erection test is performed at the time of surgery.
If an infant has a normal-appearing meatus but with a "natural circumcised" look, the possibility of chordee should be considered, and the patient should be evaluated by a pediatric urologist before circumcision.
The surgical goals of hypospadias repair are to correct chordee if present, bring the urethral meatus up to the distal terminal position in a conically configured glans to provide an appropriately directed urinary stream, and achieve satisfactory cosmesis.

Instead of being located at the tip of the penis, the meatus may be in various locations on the bottom or underside of the penis (see figure 1).

 Urology: Hypospadias
Usually, hypospadias is associated with an incomplete foreskin such that this condition is usually recognized when a newborn is examined.
However, in more severe cases, two and sometimes three or more surgeries may be needed.
Hypospadias is also associated with penile curvature in which the penis curves in a downward direction.
http://www.childrensmemorial.org/depts/urology/hypospadias.asp

 Center for Reconstructive Urology: Hypospadias
Boys with hypospadias may also have a downward curvature of the penis with erections (a condition called chordee) and a "hooded" foreskin, where the foreskin is present along the top of the penis but not along the bottom.
Surgery for hypospadias moves the urethral opening to the tip of the penis, its normal location, and corrects downward curvature.
The opening of the urethra appears along the undersurface of the penis instead of the tip of the head of the penis.

Hypospadias in Children - Keep Kids Healthy
The surgical repair is usually performed early, when your child is between six months and 12 months of age.
A hypospadias may also be associated with a chordee, or a ventral (downward) curvature of the penis.
In these cases, unless the child is being circumcised, the hypospadias will probably not be discovered until much later in life when his foreskin begins to naturally retract.
