 Gregory - Chapter 2
Dilation of the aorta may be caused by concussion of the ninth to twelfth thoracic spines or transverse processes.
To dilate the vascular supply to the spinal cords we apply vigorous but intermittent spinal concussion to the eleventh thoracic spine or transverse processes, which will cause a dilation of the blood vessels of the spinal column and this measure is very helpful in the treatment of paralysis.
Concussion of the eleventh dorsal spine or stimulation by paravertebral nerve pressure or the sinusoidal current will cause dilation of the intestines, and this is an excellent measure in the treatment of spastic constipation and in the treatment of appendicitis.

Dilation and evacuation (D&E) for abortion
Dilation and evacuation (D&E) is a surgical procedure.
Dilating the cervix reduces the risk of any injury to the cervix during the procedure.
If the procedure is done in an operating room, you could receive a spinal anesthesia injection into the fluid around the spinal cord, which numbs the area between your legs, or general anesthesia, which makes you unconscious.

Examples of such medical procedures in human patients include, but are not limited to, endometrial (uterine) biopsy, cervical dilatation and curettage, and cervical dilatation and evacuation procedures.
The inflatable dilation device according to claim 1, further comprising: a means for delivering a drug to the patient's orifice.
In addition, by maintaining a single and continuous penetration of the cervix, the physician does not have to worry about reflex closure of the cervix that typically occurs when a conventional cervical dilation device is removed from the patient.

Finally, with a database of such accurate dilation curves it is thought that better decisions can be made on interventions because one has a better understanding of the distribution of dilation behavior among different patients or patient groups.
Although there have been numerous historical attempts to increase the accuracy and automate the measurement of cervical dilatation, IPF is not aware of any commercially available, objective monitoring system for cervical diameter.
Given the subjective, intermittent nature of the measurement, it is not surprising that there is a great deal of variability in the measurement of cervical dilatation.

 Normal Labour
It is a clinical description relating to the recovery phase immediately following the completion of the third stage of labor, during which careful observation of the patient is required to ensure that the uterus does not relax and that bleeding does not occur.
In clinical practice the acceleration and deceleration phases are not readily recognized unless frequent vaginal examinations are performed.
While the uterine of labor are usually painful, they are sometimes only mildly painful, particularly in the early stages of labour.

eMedicine - Cervical Ripening : Article by Jodie Rai, MD
Induction of cervical ripening is critical to successful induction of labor in a pregnant patient whose cervix has not gone through the ripening process.
An associated change with the cervical ripening process is an increase in cervical decorin (dermatan sulfate proteoglycan 2), which leads to collagen fiber separation.
All of these changes are associated with the cervical ripening process.

It is used to conserve an already existing pregnancy and is considered a surgical procedure.

 Cervical Lips
To minimize the chance of cervical tearing, manual dilation should be fairly gentle, the cervix should continue to be assessed after the procedure, and the mother should not push until she feels the descent of the baby.
I think of the 'swollen cervix' in the mother as similar to the 'caput' in the fetus; both are caused by 'extreme' pressure' and some people's bodies are no doubt more susceptible to forming edema in the labor process.
I usually suspect either a nuchal hand/arm changing the angle of the approach, or with multips with lax abdomens, simply that the abdominal muscles are not holding the baby's body at the correct angle.

 Cervical Check
If cervical exams show no progress (no dilation) for several hours, some mothers become frustrated and concerned for no reason.
By feeling the position of the bones on the top of baby's head, the attendant can determine which direction the baby is facing (to the front, back or leaning to a side).
The station of the baby refers to the progress of the head through the pelvis.

eMedicine - Preterm Labor : Article by Edward R Newton, MD
The most common etiologies for cervical injury are elective abortion, surgeries to treat cervical dysplasia, and injury occurring at delivery.
However, the incidence of preterm birth and cervical incompetence is increased 200-300% after preconceptual surgical treatment (eg, cold knife cone, cryoconization, laser cone, LEEP) of cervical intraepithelial neoplasia (CIN).
If contractions are present without cervical change, management options include continued observation for cervical change for a total of 23 hours or therapeutic sleep (morphine sulphate 15 mg SQ).

American Family Physician: Effect of epidural analgesia on cervical dilation and labor - adapted from Obstetrics and ...
Women who received epidural analgesic had slower rates of cervical dilation and a longer second labor stage than women in the control group.
The authors conclude that epidural analgesia reduces the ability of the cervix to dilate but that myometrial activity in women who receive epidural analgesic can be maintained with oxytocin.
The authors urge clinicians to consider these effects when making decisions about the use of epidural analgesic in pain relief and management of labor.

Nevertheless, physicians should remember the conclusions of this study when caring for patients who are in preterm labor with advanced cervical dilation.
This study strengthens support for the use of a more aggressive treatment protocol for preterm labor with advanced cervical dilation.
You may be able to buy some time for the fetus by using such an aggressive protocol and by monitoring both mother and fetus carefully.

Childbirth - Wikipedia, the free encyclopedia
A woman is considered to be in labour the moment she develops regular painful uterine contractions, which are accompanied by changes of her cervix, these primarily being effacement and dilation.
However, as the cervix finishes its dilation, most mothers experience confusion or bouts of self-doubt.
During this stage, the expectant mother typically goes through several emotional phases.

 Active Labor
During early labor (latent phase of stage 1), cervical dilation tends to happen very slowly.
The physician’s expectation for progress during this time is 1 cm dilation per hour; if you’re dilating at less than half that rate, a physician may prescribe pitocin.
It’s possible to feel discouraged at this point, and feel like they’re not even halfway there.

 Protraction & Arrest Disorders
"Our data suggest that most women enter active labor at different times, mostly between 3 cm and 5 cm dilation, and even in the active phase the speed of progression varies from person to person," he further explained.
A laboring woman, who has a slower than normal rate of cervical dilation, is said to have a protraction disorder.
Protracted disorders refer to a series of events including protracted active phase dilation (slower than normal rate of cervical dilation) and protracted descent (delayed descent of the fetal head in the active phase).

The weakness can result from a number of conditions, most due to prior injury to the cervix or resulting from an inherited physical condition of the cervix.
Risk factors for an incompetent cervix are: a history of incompetent cervix with a previous pregnancy, surgery, cervical injury, DES (diethylstilbestrol) exposure, and anatomic abnormalities of the cervix.

 Epidural Prolongs Labor
The findings "suggest that early epidural placement and less-advanced cervical dilation on admission appear to be the most important predictors of prolonged labor," wrote lead study author Dr. Fergal Malone, of the Tufts University School of Medicine in Boston.
One recent study showed that anesthesia causes a slower rate of cervical dilation, an increased need for oxytocin -- the hormone that causes uterine contractions -- and a higher Cesarean delivery rate.
"This would suggest that it is not the presence or the epidural that predisposes a women to a prolonged labor, but the cervical dilation at the time of the epidural placement may be important," he concluded.

This is because increased space in the pelvis will decrease pressure of the head on the tissues surrounding the cervix, and the circulation should improve so as to reduce the swelling.
