Cardiac output - Wikipedia, the free encyclopedia
This technique uses ultrasound and the Doppler effect to measure cardiac output.
This advanced technique was developed by NASA, it measures changing resistance in the chest as the heart beats to calculate cardiac output.
This technique has progressed clinically (often called BioZ, as promoted by the leading manufacturer in the US) and allowing low cost, non-invasive estimations of cardiac output and total peripheral resistance, using only 4 skin electrodes, with minimal removal of clothing in physician offices having the needed equipment.

Critical Care Full text Why measure cardiac output?
Thus, in some patients, measurement of cardiac output is indicated as an aid to prognosis, diagnosis and to monitor the adequacy of therapy.
Thus, at least in some patients, measurement of cardiac output is indicated as an aid to prognosis and diagnosis, and to monitor the adequacy of therapy.
Thus, the use of cardiac output measures as single variables in defining disease and response to therapy require very large differences to reflect actual changes and, outside of simultaneous oxygen consumption measures, often give little insight into the mechanisms responsible for hemodynamic instability and circulatory shock or their response to therapy.

 CARDIAC OUTPUT
By comparing a pigeon and a trout, we can determine methods animals use to increase their cardiac output, as well as the volume of oxygen consumed.
Unfortunately, using Fick’s principle requires can be misleading in animals where arterial and venous blood may mix, such as reptiles and amphibians, or in animals that uptake a considerable amount of oxygen through their skin.
Organisms can improve their oxygen consumption by manipulating functions that increase their cardiac output.

Manipulating Cardiac Output and Venous Return Curves
Cardiac output and right atrial pressure are variables that are often used clinically to assess cardiovascular function.
Once again, this does not mean that it does not change cardiac output; it only means that it does not change the position of the cardiac output curve.
The manipulation of the curves depends on systematically figuring out: first, how an intervention will affect the cardiac output curve, and then how it will affect the venous return curve.

eMedicine - Perioperative Cardiac Management : Article by Davinder Jassal, MD, FRCPC
During the 1970s, several studies reported a 30% risk of reinfarction or cardiac death for patients undergoing surgery within 3 months of an MI, 15% when surgery was performed 3-6 months after an infarction, and 5% when the operation was performed 6 months later.
As a result of the stress of the surgical procedure, circulating catecholamine levels are elevated, thereby increasing the risk of ventricular ectopy.
A preoperative cardiac risk assessment involves obtaining a thorough history and physical examination, along with a baseline electrocardiogram.

Video Transcript - The Determinants of Cardiac Output
The first set is applicable when the heart is in myocardial energy excess and includes mean vascular pressure -- which includes fluid volume and system compliance -- and inlet impedance -- which is the product of a resistance point and a distensible bed upstream from that resistance point.
Therefore, I believe we are safe in presuming that the human heart is a non-sucking, intermittent outflow, continuous inflow pump.
To answer that question, you must first understand that until this point we've been examining conditions where the pump is exerting excess energy above what was used for circulation.

 Cardiac Output
The cardiac output represents the volume of blood that is delivered to the body, and is therefore an important factor in the determination of the effectiveness of the heart to deliver blood to the rest of the body, (i.e., determining heart failure, inadequate circulation, etc).
Cardiac Output is usually measured using the Fick Principle, which relates the cardiac output of the patient to the oxygen consumption, or by thermodilution, in which cold saline is injected into the right atrium and changes in the temperature in the pulmonary artery are recorded.
Based on this information, what is this patient's cardiac output?

BACKGROUND: Measurements of cardiac output with the thermodilution technique add to data for clinical decision making and therefore must be valid and reliable.
However, the results of studies on the accuracy of values obtained with room-temperature and iced injectates, especially in patients with high or low cardiac output, have been conflicting.
CONCLUSION: The findings support the practice of using room-temperature injectate to measure cardiac output in patients with low cardiac output.

It is the change in cardiac output, up or down, that allows the practitioner to determine the effects of therapy and disease and not the absolute value but to accurately detect changes, the output measurement errors must be consistent.
Thermal dilution cardiac outputs are affected by the phase of respiration, particularly during mechanical ventilation and should thus always be measured at the same point in the respiratory cycle (normally end-expiratory) where the effect of breathing (either spontaneous or mechanical) is least.

 Determinants of cardiac output:
In physiological terms, afterload can be defined as 'The sum of all those forces which oppose ventricular muscle shortening during systole' - although in a clinical sense it is probably more useful to consider systemic vascular resistance as the appropriate measure.
In isolated cardiac muscle, there is an inverse relationship between afterload and the initial velocity of shortening of the muscle (Figure 3).
In contrast, if myocardial function is severely depressed, cardiac output may become crucially afterload-dependent as illustrated in Figure 4.
