Just a quick additional note to say that sometimes, for people who are having symptoms such as you describe, that good-quality orthotic devices, sometimes with an intrinsic forefoot posting, can hold the forefoot in such a manner that the repetitive jamming of the 1st MTPJ either does not occur at all, or is markedly lessened.
