 Atosiban
A total of 246 patients were randomized to receive atosiban and 246 to placebo; the primary endpoint was the time from the start of study drug to delivery or therapeutic failure.
The efficacy was analyzed by intention to treat and the safety according to the drug that the patient in fact received.
The incidence of adverse effects in both groups was comparable (thoracic pain, tachycardia and fetal distress) except for injection site reaction that was higher in the atosiban group.

 New drug
Atosiban, originally piloted in 1987 to treat painful periods, is being heralded as a breakthrough in the development of tocolytics &; drugs used to delay labour — because it has no alarming side-effects.
Premature births account for more than two thirds of all neonatal fatalities worldwide, and those babies who survive a preterm delivery are often prone to disabilities that may dog them throughout their lives.
It was found to be as effective, but much less likely to cause cardiovascular problems.

 Data Sheet
As an antagonist of oxytocin, atosiban may theoretically facilitate uterine relaxation and postpartum bleeding, therefore, blood loss after delivery should be monitored.
Admission to a neonatal intensive care unit (NICU) was similar for both treatment groups (approximately 30%), as was length of stay and ventilation therapy.
The onset of uterus relaxation following atosiban is rapid, uterine contractions being significantly reduced within 10 minutes to achieve stable uterine quiescence (≤ 4 contractions/hour) for 12 hours.

Clinical Significance of Atosiban in the Treatment of Preterm Labour
The favourable safety profile of atosiban in combination with equivalent tocolytic effectiveness would appear to represent an advance over current tocolytic therapy.
Neonatal outcomes were found to be comparable in both treatment groups.
What does atosiban offer for future clinical practice?

Oxytocin antagonists: clinical and scientific considerations -- Thornton et al. 86 (2): 297 -- Experimental Physiology
Although it may be an effective tocolytic, a beneficial effect on perinatal outcome has not been demonstrated.
Recently, an oxytocin antagonist (atosiban) has been introduced into clinical practice in Europe.
The reasons are numerous and complex, but they include a failure to understand the mechanism(s) of preterm labour, the multitude of different causes, the difficulty in diagnosis and the problems of outcome measurement in clinical trials.

OBLink Preterm Labor Resource - Head to Head: Nifedipine versus Atosiban - transcript
Nifedipine is a drug that affects all vasculature, in fact that’s why we use it in obstetrics for pre-eclampsia, etc. And so it still has some side effects: flushing, headache, tachycardia, but they’re not usually a major problem at least in well people.
The other side of the equation in side effects is always the baby.
Atosiban is a drug that’s specific for uterine muscle and very little of it crosses the placenta so it has very few side effects.

p144 : Extracorporeal Perfusion of the Porcine Uterus
The objective of the study was to establish an experimental model for the examination of the uterine function, using the extracorporeal perfusion of the swine uterus.
In order to examine viability and responsiveness to hormones the effect of Oxyroxin, Prostaglandin E2 and various tocolytic drugs (Fenoterol, Terbutaline, Ritodrine, Alcohol, Atosiban, Aspirin, Glyceroltrinitate, Verapamil and Propofol) were tested.
Our results demonstrate clearly that the perfused swine uterus can be maintained in a normal functional status and thus is a suitable experimental model to study the physiology of the pregnant and non pregnant uterus.

Also, 23% of women given atosiban and 31% on placebo had at least one subsequent iv therapy for recurrent PTL.
Therefore, atosiban prolonged the time interval to first recurrence of PTL and reduced the need for subsequent iv therapy.
These findings, which were dependent on gestational age, suggest a role for atosiban in the treatment of PTL.

New drug for delaying premature birth marketed
The proportion of women who remained undelivered and not requiring alternative treatment within seven days of therapy initiation was 59.6 per cent for atosiban and 47.7 per cent for the beta-agonist.
The manufacturer, Ferring, reports that phase III clinical trials showed atosiban to be superior to a beta-agonist.
The drug has also been shown to inhibit the effects of vasopressin by binding to its receptor.

 Obstetrics Products
The active ingredient, atosiban, is an oxytocin antagonist that blocks the action of oxytocin and is able to prevent uterine contractions and relax the uterus.
TRACTOCILE (atosiban) is a novel treatment indicated to delay imminent pre-term birth, which is the main cause of death and disability in new-born infants.
Ferring is dedicated to advancing the treatment of pre-term labour supporting a reduction in the number of premature births and the high risk of death and serious disability associated with it.

Our results do not support the contention that vasopressin is involved in the etiology of dysmenorrhea, plasma concentrations of vasopressin being similar in dysmenorrheic women and controls, and the vasopressin antagonist Atosiban having no effect on menstrual pain, intrauterine pressure or uterine artery pulsatility index in dysmenorrheic women.
We compared menstrual pain, uterine contractility and blood circulation, and plasma concentrations of vasopressin and prostaglandin F
metabolite in women with versus without primary dysmenorrhea, and determined the effects of a vasopressin antagonist, 1-deamino-2- D -Tyr(OEt)-4-Thr-8-Orn-oxytocin (Atosiban), on these parameters.

Infusion with an oxytocin receptor antagonist delays parturition in a marsupial -- Renfree et al. 108 (1): 131 -- ...
The oxytocin receptor antagonist [1-deamino-2-D-Tyr-(OEt)-4-Thr-8-Om]-oxytocin (Atosiban) is a specific antagonist of both mesotocin- and oxytocin-induced myometrial contractions in late pregnant tammars in vitro.
In both the 3 day and 7 day infusion groups, all 15 control animals were pregnant and gave birth within the normal time (day 26.75 +/- 0.20, mean +/- SEM), during the infusion of saline.
Continuous intravenous infusion of Atosiban (1 mg kg-1 day-1) for 3 or 7 days from day 24 of the 26.5 day gestation significantly delayed births.

Atosiban - Wikipedia, the free encyclopedia
Chemically, it is a modified form of oxytocin that inhibits the action of this hormone on the uterus, leading to a cessation of contractions.
Atosiban ( Tractocile®) is an inhibitor of the hormone oxytocin, and is used in premature labor.

In this paper we describe a computer simulated docking pertinent to affinity of an oxytocin antagonist atosiban towards OT receptor, versus vasopressin V1a and V2 receptors.
Conversely, oxytocin antagonists blocking uterine OT receptor can suppress uterus contraction.
