Asymptomatic Carotid Stenosis -- How Much Difference Does Surgery Make? - Journal Watch (General)
Five randomized trials involving 2,440 asymptomatic patients with carotid stenosis of 50 percent or more were analyzed.
However, surgical patients had a 2.4 percent risk for stroke or death in the 30 days after randomization, compared with 0.4 percent for the medically treated group.
An accompanying editorial points out that 50 people would have to be operated on to prevent one stroke and that we need better ways of identifying which asymptomatic patients are at higher or lower stroke risk before recommending surgery.

Additionally, lymphocyte responses were quite variable, and in some asymptomatic carriers the lymphocyte proliferation and IFN-γ production were similar to those found in patients with myelopathy.
Immunoproliferation assays, cytokine levels of unstimulated cultures, and flow cytometry analysis were used to evaluate the studied groups.
A great variability of immune response observed in about 40% of HTLV-I infected subjects was not reflecting a temporary situation, since the IFN-γ levels were relatively constant within an individual over time (eight months).

However, he said it is important to note that this study did not find that exercise testing provided additional prognostic information in those with 10-year-CHD risk less than 10 percent.
"Our results suggest that exercise testing may be of benefit in asymptomatic men with intermediate to high risk," said Gary J. Balady, M.D., a professor of medicine at Boston University School of Medicine and an investigator with the Framingham Heart Study.
This is the first study to evaluate exercise testing among asymptomatic people relative to their predicted coronary heart disease (CHD) risk using the Framingham risk score.

However, the researchers cautioned that the study is not a prospective, randomized trial -- something that may be difficult to perform in this relatively rare disease -- and argued that more cohort studies are needed before the wait-and-see approach is widely adopted.
Autoimmune hepatitis "is generally associated with a favorable outcome," Dr. Feld concluded, and "they appear to do well without immunosuppressive therapy at least for as long as they remain asymptomatic."

"Pegylated interferon leads to higher sustained viral response rates in symptomatic patients with acute HCV infections, but the treatment of asymptomatic HCV is not yet established," said Sanaa Kamal, MD, professor of medicine, Harvard University, Boston, Massachusetts.
Most patients with acute HCV infection are asymptomatic, but these acute infections evolve into chronic infections in the majority of patients.
Initiation of therapy at 8 or 12 weeks leads to a higher sustained virologic response rate than at 20 weeks, particularly with treatment with interferon plus ribavirin, she added.
