eMJA: Hugh & Hugh, Appendicectomy -- becoming a rare event?
Appendicectomy for acute appendicitis and for other conditions: an epidemiological study.
Incidental appendicectomy was clearly overdone in the past, but it is justifiable in younger patients and should not be abandoned.
Donnelly et al raise the question of whether rural doctors have taken full advantage of these changes, but provide valid reasons why rural approaches to appendicitis appropriately may differ from city practice.

General anaesthesia is used to put the patient to sleep and to relax the abdominal muscles.
There are two surgical techniques that are frequently used, open appendicectomy or laparoscopic appendicectomy.

 Appendicectomy and ulcerative colitis
A case-control study was conducted, comparing appendicectomy rates between 110 patients with UC (group 1) and 136 patients attending an orthopaedic clinic free from UC (group 2).
This change may have a role in preventing the development of ulcerative colitis in a vulnerable group of patients.
They found that 1/174 patients suffering from UC had a history of appendicectomy compared with 41/161 in the control group.

eMJA: Donnelly et al, Appendicectomy in Western Australia: profile and trends, 1981-1997
The decline in the incidence of appendicectomy in WA from 1981 to 1997 is consistent with trends in other industrialised countries and most likely reflects a change in attitude to the use of the procedure, coupled with improvements in diagnostic technology.
The authors raised the concern that appendicectomy without acute appendicitis was much more common in women than men, questioning the appropriateness of the use of the procedure.
While the physiological role of the appendix is unclear, it may have surgical potential in reconstructive urology and the management of faecal incontinence.

The technique of appendicectomy may vary from surgeon to surgeon or from center to center, starting from skin incision to the ligation and invagination of appendicectomy stump, and so on.
During appendicectomy, some surgeons do simple ligation of the appendix stump while others still prefer to invaginate it by a purse-string suture or Z-stitch.
Fascinating new operative techniques have emerged and replaced the conventional procedures but in the case of appendicitis we are still relying on the traditional method of appendicectomy in our country.

With the advent of minimally invasive surgical techniques, and their established benefits, particularly in women of childbearing age, there has been an increase in the number of diagnostic laparoscopies and laparoscopic appendicectomies performed within our surgical practice.
The authors reviewed the use of microbiological testing in emergency appendicectomy within their own hospital to assess the contribution of swabs taken during surgery to subsequent patient management.
There is also extensive research regarding the appropriate antibiotics for prophylaxis and suggested protocols for the therapeutic use of anti-microbials.
