Cardiomyopathy as a Late Effect of... - Association of Pediatric Oncology Nurses
While there is a clear association between the development of cardiomyopathy and treatment with anthracycline chemotherapy, there are a variety of risk factors that influence the actual incidence of cardiomyopathy.
Thus, cardiac monitoring is imperative not only while the patient is receiving anthracycline therapy, but for years afterward.
Survival rates for children and adolescents treated for childhood cancer have improved due to advances in chemotherapy, radiation therapy, surgical intervention, and supportive care.

The theory, though, that histamine plays a role in anthracyclines cardiac lesions is not supported by the fact that in other conditions, where there is increased exposure to histamine, such as in anaphylactoid reactions or in mastocytosis, there are no cardiotoxic effects similar to those from anthracyclines.

Nearly two-thirds of patients in the anthracycline group experienced alopecia compared to 24 percent in the CMF group.
By contrast, 18 patients out of 21 (86 percent) who received anthracycline experienced a complete response, a figure that was statistically significant.

 Thesis_1
Anthracycline targeting seems to be one realistic possibility, but in spite of early optimism, this approach has proved extremely difficult.
There is no doubt, however, that the anthracyclines will be the drugs of choice for some while yet, but much more basic research is required before we truly understand how they function in the growing tumor and how new derivatives with improved therapeutic properties can be synthesized.
In particular, the iron-dependent generation of reactive free radicals as an explanation of anthracycline cardiotoxicity continues to be controversial and is rejected by many researchers.

Where indicated, NADPH-dependent conversion of anthracyclines to secondary alcohol metabolites was assayed in cytosolic fractions of human myocardial samples, precisely as reported ( Licata et al., 2000 ; Minotti et al., 2000).
This study lends support to the concept that secondary alcohol metabolites may be important determinants of cardiotoxicity induced by anthracyclines.
This suggested that alcohol metabolites were important determinants of negative inotropism induced by anthracyclines.

 Late Effects: Heart
Note: Linda Z points out one more heart problem related to treatment, ant that is the potential for developing sudden and life threatening arrhythmias because of scarring to the clump of nerves that are responsible for monitoring the rhythm of the heart.
Chemotherapy (specifically, the anthracyclines) and radiation can affect the heart years after treatment.
It's difficult for us to hold our kids back, especially when they seem very healthy and have a lot of physical endurance, and when they have had echocardiograms at the end of treatment which indicate that they do not have heart damage.

Better Breast Cancer Therapy: Making Anthracyclines More Effective
TERRY P. MAMOUNAS, MD: There's very convincing evidence that adjuvant chemotherapy in general will improve survival, and certainly that's including evidence that anthracycline-containing therapy is actually more effective than non-anthracycline-containing therapy.
KATHLEEN PRITCHARD, MD: I think the biggest window of difference is for the cardiac effects, where you can give almost twice as much, milligram per milligram, epirubicin as you can adriamycin before seeing the same kind of cardiac problems.

Biological Barriers Contributing to Anthracycline Resistance
An understanding of the biological mechanisms contributing to treatment failure with current regimens is necessary to develop more effective therapeutic approaches for acute leukemia.
Correlative studies suggest that some of the recognized mechanisms have prognostic relevance and, therefore, may contribute to clinical drug resistance.
Other variables possibly influencing response to this type of treatment approach include the prevalence of P-gp-positive cases in each treatment arm, the ability to achieve and sustain effective chemomodulator blood levels and the presence of non- MDR mechanisms affecting anthracycline resistance.

In this retrospective trial, Dr R. Kim, and colleagues, Department of Surgical Oncology, Research Institute for Radiation Biology and Medicine, Hiroshima University, Japan, evaluated the response and survival benefits of combining primary neoadjuvant systemic therapy including an anthracycline-based regimen with adjuvant chemotherapy in 25 patients with stage IIIB, locally advanced breast cancer.
The 5-year survival rate was 69.7% in patients treated with neoadjuvant chemotherapy compared with 77.8% in patients treated with adjuvant anthracycline-based chemotherapy.
Primary neoadjuvant systemic chemotherapy regimens including anthracyclines may offer a potential survival benefit when administered with adjuvant chemotherapy since they can decrease local recurrences and distant metastases in patients with locally advanced stage III breast cancer, write Japanese researchers.
