eMedicine - Angiodysplasia of the Colon : Article by John Godino, MD
Right hemicolectomy for angiodysplasia is second-line therapy after endoscopic ablation, if repeated endoscopic coagulation has failed, if endoscopic therapies are not available, and for life-threatening hemorrhage.
Angiodysplasia that presents with acute hemorrhage can be controlled effectively with angiography, although it seldom is needed.
Internationally: No wide-scale studies to determine the incidence of angiodysplasia have been conducted, but the incidence probably is similar to that in the United States.

 Angiodysplasia
Definitive cure requires surgery, however, as either angiographic occlusion or endoscopic therapy will provide only temporary relief of the bleeding in most instances.
This will greatly facilitate location of the exact site of the lesion during surgery.
If surgery is considered following angiographic demonstration of angiodysplasia it is advisable to leave a small catheter in place at the end of the procedure.

Hormone Therapy for GI Bleeding from Angiodysplasia - Journal Watch (General)
During an average follow-up of 14 months, there was no significant difference between groups in the incidence of rebleeding (39 percent of hormone-therapy recipients vs. 46 percent of placebo recipients) or mean number of bleeding episodes per year (0.7 vs. 0.9).

Because gastrointestinal bleeding from angiodysplasia is such a vexing problem, it may be worth studying whether differing patient populations account for the varied outcomes associated with hormone therapy, and whether hormone therapy is effective in certain subgroups.

Angiodysplasia - Definition of Angiodysplasia by Webster's Online Dictionary
Development is related to age and strain on the bowel wall, which is thought to influence the caliber change and proliferation of the vascular tissue.
Although the lesions can be notoriously hard to find, the patient usually is diagnosed by endoscopy.

However, patients who have bleeding angiodysplasia despite having had colonoscopy, angiography, or surgery, are likely to have recurrent bleeding in the future.
Angiodysplasia of the colon has been seen in about 3% of "nonbleeding persons" and in about 5% of patients evaluated for blood in the stool, anemia, or hemorrhage.
Otherwise, the symptoms are the same as any other condition that causes bleeding from the gastrointestinal tract, with the caveat that this disease does not cause pain.

During therapy, the mean monthly transfusion requirement of packed red blood cells in the treated group was not significantly different from that found before therapy (1.5 vs. 2.2 units, NS) or from that of the control group (1.5 vs. 1.6 units, NS).
Question: Does hormonal therapy have any benefit for bleeding angiodysplasia?

 Echocardiography
The authors conclude that the low prevalence of aortic valve stenosis in patients with gastrointestinal angiodysplasia (1/59 or 1.6%) in this retrospective study argues against the association of gastrointestinal angiodysplasia and aortic valve stenosis.
Echocardiographic findings were reported in a retrospective study of 83 patients who underwent gastrointestinal endoscopy and were found to have angiodysplasia.
However, both aortic stenosis and angiodysplasia are common in the elderly, and the association between the two may be coincidental, rather than causal.

Colonic angiodysplasia [CJS - August 1998]
Although colonic angiodysplasia may be associated with concurrent medical disease processes, such as aortic stenosis (Heyde's syndrome), it is generally considered to be a process of senescence.
The pathogenesis of angiodysplastic lesions is widely believed to be mechanical: chronic intermittent submucosal venous obstruction, leading to vascular ectasia.
Blood loss can be occult, presenting as chronic iron deficiency anemia, or it can present as hematochezia or melena.

She had however been using acetosalicylic acid regularly, and it is possible, that the angiodysplasia could have been bleeding intermittently, giving an explanation to the anaemia.
She had a hiatal hernia, and inside the hernia this little angiodyplasia, probably an innocent finding.

colonic angiodysplasia - General Practice Notebook
Colonic angiodysplasia is a common cause of acute or chronic rectal bleeding and iron deficiency anaemia.
Angiodysplasias are tiny - 1-5 mm in diameter - hamartomatous capillary lesions in the colonic wall which produce bleeding out of proportion to their size.
They are believed to be acquired, possibly as a result of tension on the veins where they pass through the muscularis.

 gastric angiodysplasia
A giant gastric angiodysplasia causing recurrent gastrointestinal bleeding was found in a 51-year- old woman who had been treated with repeated blood transfusions for severe anemia.
This giant lesion had spontaneous oozing of fresh blood and was suspected to be a gastric angiodysplasia.
At first, no active bleeding source was seen on upper or lower GI endoscopy.

posted 06-17-2003 02:15 PM I am a female with chronic angiodysplasia (leaking veins in the right colon.) When I have weeks of slow bleeding, I get very anemic.
And I do have an appointment with him again next week.
Two weeks ago my blood count was down to 9.6 and the hematocrit was 30.
