Improving Sexual Function with Androgen Therapy
Androgens have actions on multiple areas of the body including bone, muscle, skin, liver, kidney, brain, immune system, bone marrow and genital organs.
All patients on androgen therapy should have quarterly follow-up in the first year.
Based upon available clinical trial data, and the 4 year experience at the Center for Sexual Medicine, these side effects are infrequent if androgen levels are maintained within normal physiological ranges and the delivery system of androgen administration is via topical testosterone therapy.

 Androgen Insufficiency in Men
Androgens affect sexual desire, bone density, muscle mass and strength, adipose tissue distribution, mood, energy and psychological well-being.
Symptoms of androgen insufficiency often improve by taking androgen therapy (formerly known as androgen replacement therapy or androgren supplementation).
If you are concerned with any physical changes which might occur after you start androgen therapy, you are welcome to contact one of our health professionals at 617 638-8576 or csm@bmc.org.

 Androgen Suppression for Prostate Cancer
In regards to androgen suppression therapy in localized prostate cancer, physicians at the meeting expressed differing detailed views of the precise stage at which to initiate hormonal therapy, optimal duration of treatment with hormonal therapy, and timing in regards to radiation therapy.
Neoadjuvant and adjuvant androgen therapy appears to improve outcomes of patients with intermediate or high-risk localized prostate cancer who are being treated with radiation therapy.
Although outcomes may be improved with the use of adjuvant androgen deprivation in localized prostate cancer patients who are being treated with primary radiation therapy, acute and long-term toxicities need to be weighed against these outcomes.

 Androgen replacement therapy
The therapeutic efficacy of androgen replacement is assessed by monitoring the patient’s clinical and serum testosterone responses.
The main use of androgen replacement therapy is in the management of men with hypogonadism.
In prepubertal boys and men with either primary or secondary hypogonadism, androgen replacement therapy is indicated to stimulate and sustain normal secondary sexual characteristics, sexual function, and behavior.

Intermittent Androgen Suppression Delays Progression to Androgen-independent Regulation of Prostate-specific Antigen ...
Indeed, an increasing serum PSA following androgen ablation therapy in men with prostate cancer is the first sign of androgen-independent prostate cancer progression (6).
In contrast, progression to androgen independence nearly always occurs following androgen ablation therapy for patients with prostatic carcinoma.
Whether IAS will enhance progression-free survival or overall survival in patients with prostate cancer is unknown, although the possibility of an improved quality of life and outcome has been considered by others [31].

 Androgen Insensitivity Syndrome
Normal responsiveness to androgen after birth does not prove that it was normal before birth.
A common practice is to remove the testes after puberty when feminization of the affected individual is complete.
However, the great majority of such negative searches very probably reflect a "timing" problem; in other words, the acquisition of normal testosterone synthetic capacity or normal androgen responsiveness is delayed beyond the critical periods for normal external and/or internal male genital differentiation.

Androgen deprivation therapy produces distinct histopathologic changes in both neoplastic and nonneoplastic prostate tissue.
In 1996, the Canadian Urologic Oncology Group (CUOG) published the results of a randomized study on 213 patients, of which 112 were treated with 12 weeks of cyproterone acetate at 300 mg/d prior to surgical therapy.
Three hundred and three patients were enrolled in the study and randomized to RP plus NAD with an LHRH agonist and flutamide (149 patients) or surgery alone.

Hypertrichosis : Hypertrichosis basically means "excess hair growth." Many physicians however use the word "hypertrichosis" to refer to the excess growth of vellus (fine and soft) hairs, as opposed to hirsutism (the excessive growth of thick dark hairs in a male-like pattern).

A trial was classified as higher quality when it was double-blinded (except when orchiectomy was one of the treatments) and reported outcomes based on an intention-to-treat analysis.
The meta-analysis found a significant difference in 5-year overall survival in favor of radiation therapy plus continued androgen suppression compared to radiation therapy alone (hazard ratio = 0.631, 95 percent CI = 0.479 to 0.831).
The meta-analysis found no significant difference between immediate primary hormonal therapy and deferred therapy for survival at 5 years (hazard ratio = 0.914; 95 percent CI = 0.815 to 1.026), although the 95 percent confidence interval approaches statistical significance.

One of the central questions in the study of androgen-independent prostate cancer concerns the point at which androgen-independent cells arise.

 Androgen Receptor
The androgen re-ceptor may be amplified and therefore may be activated by reduced levels of dihydrotestosterone.
During androgen-independent progression, prostate cancer relies on various cellular pathways, some involving the androgen receptor and others bypassing it.
In the pathways that bypass the androgen receptor, the loss of PTEN reverses the inhibition of the phosphatidylinositol 3-kinase (PI3-K)–Akt pathway, permitting activated Akt to phos-phorylate Bad.

MedlinePlus Medical Encyclopedia: Congenital adrenal hyperplasia
Congenital adrenal hyperplasia refers to a group of inherited disorders relating to the adrenal glands, characterized by a deficiency in the hormones cortisol and aldosterone and an overproduction of androgen.
Without these hormones, steroids are 'diverted' to becoming androgens, a form of male sex hormones.
The defect involves a lack of an enzyme needed by the adrenal gland to make the major steroid hormones of the adrenal cortex: cortisol and aldosterone.
