The appearance may be quite ambiguous, describable as female genitals with a very large clitoris and partially fused labia, or as male genitals with a very small penis, completely open along the midline ("hypospadic"), and empty scrotum.

Doctors Advise Caution in Ambiguous Genitalia Surgery in Babies
A new study suggests that, in cases of ambiguous genitalia, feminizing the child with clitoral surgery may lead to a lifetime of sexual problems.
The women's sexual functioning could be hindered by shyness -- or by behavior problems related to the gender in which she was raised, writes Froukje M E Slijper, MD, of Erasmus MC/Sophia Children's Hospital in Rotterdam, in an accompanying editorial.
"Most women born with ambiguous genitalia are masculine in behavior," writes Slijper.

Among other girls, the underlying reason for the masculinization of their genitalia may be that mother had an excess of masculinizing hormones in early pregnancy that passed across the placenta.
Among girls whose external genitalia were only slightly masculinized at birth or who have undergone modern feminizing surgery, the appearance of their genitalia should not later give rise to concern.
Here, the underlying cause can usually be traced to a fault in one of the steps along the pathway of development in which the external genitalia of the early embryo normally become fully masculinized.

An Emerging Ethical and Medical Dilemma: Should Physicians Perform Sex Assignment on Infants with Ambiguous Genitalia?
Surgical treatment of ambiguous genitalia in infancy exemplifies an instance where prevailing medical wisdom, in an area of immense significance to individuals and their families, developed without any conclusive evidence that surgical intervention was appropriate.
Indeed, it is the ambiguous nature of the genitals that signal the ambiguous nature of the child’s future sexual identity and preferred gender, and that innate ambiguity cannot be masked by surgery.
These critics argue that parents of children with ambiguous genitalia would be better counseled to manage the psychosocial consequences of genital differences in childhood rather than opting for a surgical response.

Letter To The Editor: Sexual Outlook for Post-Surgical Ambiguous Genitalia Patients
Recognizing that corrective genital surgery will be undertaken on the majority of girls with ambiguous genitalia, development of a surgical procedure that will both normalize genital appearance and maintain genital sensation is ideal.
Comparing (admittedly anecdotal and by personal experience) family outcomes in the decades when clitoral surgery in girls with ambiguous genitalia was performed between 2-5 years of age and the current practice of clitoral recession within the neonatal period, far more marital conflicts (spousal abuse, separation, divorce) arose with the delayed rather than early clitoral surgery.
It is possible that such an integrated approach (medical, surgical, and psychological) will result in more positive outcomes for these individuals.
