Topic: Airway management

Encyclopedia of Nursing and Allied Health: Airway management
When a patient is critically ill and requires an artificial airway and mechanical ventilation, it is the responsibility of the healthcare professionals caring for the patient to ensure that the airway is secure.
The respiratory therapist is generally responsible for ventilating the patient with a bag-valve-mask device until reintubation (reinsertion of the airway), while the nurse gathers equipment, administers medications, and monitors the patient's pulse oximetry, vital signs, and cardiac rhythm.
The nurse and respiratory therapist are equally responsible for monitoring and managing artificial airways.

 WL97 - Upper Airway Management
Learning to use the equipment when an airway management problem presents itself is an extremely common occurrence, but it is usually an unrewarding situation that impacts negatively on the practitioner and the patient.
The postulated mechanism is that the LMA causes reflex relaxation of the LES by distention of the hypopharyngeal muscles, similar to the effect of a food bolus.
Mallampati reported a correlation between the visibility of oropharyngeal structures and the degree of difficulty of laryngeal exposure during laryngoscopy and concluded that difficult laryngeal visualization could be predicted in most cases by assessing the visibility of the faucial pillars and uvula.

 Airway Management
With difficult intubation being reported as up to 3% of airway emergencies in the emergency department1, and the incidence of failed intubation in the operating room approximately 1 to 3 per 1000 patients2, the skillful use of both traditional and alternate techniques are necessary to provide adequate care.
With regards to airway management, the history should focus on prior intubations, anesthetic history, drug allergies, and confounding illnesses that may hinder airway access.
Over the past 20 years, changes in airway management techniques have led to decreased morbidity and mortality in both the acute care and surgical setting.
